I am not a fan of this just because it is the ENTIRE name of a very popular character in a movie that JUST came out so I'm sure your daughter will have many friends named Hazel if not Hazel Grace. Not very original.
I really like Hazel. The Fault in Our Stars reference aside, I feel like every other person gives their baby girl the MN Grace these days. It's a nice enough name, but unless it has particular meaning to you, I suggest a more distinctive MN.

Hazel is in our top 2 for a girl! Love it, obviously Grace is a very popular middle name, but Hazel is not common, so it balances. I think it's classic and lovely.
I love both names. I haven't seen/read the fault in our stars though. That might deter me. But honestly, I don't think it will have any significance 10 years from now.
Hazel Faith is alright. I like Faith as a MN better than Hazel as a FN just because Hazel is somewhat trendy as far as "old lady"/color names go. But I like Hazel as a name well enough. It's also not as trendy as Violet or Scarlet.
I agree with others about Grace being overused and about Hazel Grace in particular being trendy. There's a reason it was used as a name for the main character in a recent movie.
I love Hazel but yes; all I can think is the fault in our stars. I'm also not a huge fan of "virtue" names straight out, I prefer when the name means something special instead but isn't so obvious? For instance: Hazel Johanna- God is gracious (so kind of along the same lines as grace)
Just a suggestion!
Eta; it's the full Hazel Grace that reminds me of FIOS, not just the Hazel. I've always been rather fond of the name even if it didn't make it on our girl list!
Can't stand Hazel. It's so old-lady to me, and not in a good way. Could be because my great-grandmother was named Hazel and she was always an old, old lady to me. But there are plenty of other, better, options out there.
I don't mind Grace, either as a first or middle name, but it is a bit over-used in my circles.
A couple of options: Louise/Louisa Frances Laura Hope
Its like naming a son Harry James..... you don't do it. Also, you would be naming your child off a cancer patient, which is kind of weird.
If you have to use one of them, pick one. I think Hazel is way too trendy right now (and also can't really stand it trendy or not). Grace is a lovely MN though. Ideas, Hannah Grace, Olivia Grace, Louise Grace...
What about Grace as a first name? I love the name on its own, it'd be on my short-list if I wasn't the world's biggest klutz. If klutziness is genetic I don't want to curse my daughter with an ironic name.
Hazel is nms, but it's not a bad name by any means. Also I had no idea about the movie reference, but I don't think that movie was big enough to make that a problem. Twilight on the other hand ruined so many lovely names for me...
I love it! And seriously who give a flying f*ck if it is the name of a character in a movie/book? A few years from now no one will relate it to the movie and it's not like the kid will be walking around introducing itself as hazel grace to everyone...it will be Hazel. I love it and if you love it go with it. My daughters middle name is Grace and I think it's classy.
Re: Classic or too old fashioned?
Hazel Faith?
I like Hazel Joy better than Hazel Faith, if that still works for you. If not, Hazel Faith is totally fine too

I have not read/ seen FIOS.
I honestly don't love Hazel. It does not feel pretty to me.
What about Grace as a FN?
