Milestone/Yearly Book?

I think someone posted a link here to a worksheet of sorts that they've created for use in documenting their LO's yearly birthdays. Basically you fill in like favorite food at the time, interests, etc. I can't find the link anywhere! Any help?!

I know we talked about one year chalkboards or posters, but I don't have the link. If you Google one year poster, there are plenty of images. There are a bunch of options on Etsy!
