As far as when it is time to let go, here is the advice I give my clients. It is all about quality of life. Is he in pain? Is the pain able to be kept at a manageable level with medication or other therapies such as acupuncture? Then I try to pick a few things that I know really make the animal happy. Does he live laying in a sunbeam by a window, or run when a can of wet food is open, or chase string...things like that. I try to look at those things, and when the animal doesn't enjoy those things anymore, it may be time to let go. Also when you have more bad days than good days.
Ultimately, you know him the best, and will know when it is time.

Typical ER exams can range anywhere from about $70-100 depending on the area. They should be able to provide you an estimate after they do the exam and get a feel for what needs to be done. I completely agree, it has everything to do with quality of life. Eating, drinking, activity level, etc....if all of that goes out the window despite medical intervention, it might be time.
