Scared of metformin

I'm starting metformin tonight, and I know it causes stomach issues. I am doing one a day for now and stepping up each week until I'm doing four a day. I already have stomach issues that I am always cautious about so am worried about this. Any tips I should keep in mind? I am a therapist and see clients throughout the day, mainly at their homes, and am concerned!

Me: 32  DH: 33  Married: March 2004

July 2006: started TTC
2008: HSG (normal), couple rounds of clomid through gyno
2008 - 2010: dragging my feet out of fear and procrastination
October 2010: first consultation with RE, dx PCOS and fibroids (DH slightly low count/motility)
Oct. 2010 - Dec. 2012:  In DENIAL! avoided the issue because I was scared of surgery
January 2013: returned to RE, fibroids grew significantly
February 2013: second HSG, fibroids pushed on tubes which blocked them somewhat
March 2013: MRI to determine what type of surgery may be necessary

July 29, 2013: fibroids (5) removed via robotic laparascopy
August 2013 - Nov 2013 : benched due to recent surgery

IUI #1, Dec. 24, 2013, BFN 
IUI #2, Jan. 25, 2014, BFN
IUI #3, Feb. 25, 2014  BFN
IUI #4 canceled due to lack of response to letrozole
IUI #4.1 April 28, 2014, BFN

May 16, 2014: wtf consult, start prepping for IVF in June and add injects for one last IUI in the meantime
IUI #5 started letrozole and bravelle but canceled after HSG led to new diagnosis

May 21, 2014: third HSG, tubes blocked, one at the beginning, one hydrosalpinx??
June 11, 2014: consult, approved to move on to IVF because the hydro is not completely blocked therefore allowing fluid to move through slowly rather than backwards
IVF #1 August 8, 2014 - 3dt of 2 embryos, BFN
September 17, 2014 - 4th HSG, the right tube is very patent (open!!) dye went straight through this time. Weird!
October 2, 2014 - started metformin treatment
November 14, 2014 - blood work, brought A1C down from 5.8 to 5.5

November 26, 2014 - RE finally back from vaca and reviewed my chart, no more IVFs for rest of calendar year
December 1, 2014 - Right after Thanksgiving, I called a new clinic and got in right away! Plan for IVF
December 17, 2014 - ER! 29 retrieved (!!), 16 mature, all 16 fertilized (ICSI)

IVF #2 December 20, 2014 - 3dt of 3 embryos, BFN

We are done with treatment unsuccessfully. :(

  • Ditto the ER. It does take a bit to get used to, but now I only have GI flare ups every once in awhile. It's not the greatest, but if it does what it's supposed to to help wit getting pregnant, it's worth it! Lots of luck!

  • I think I'm on the regular kind, not ER, and only had issues the first few days. No problems eating bread and sugar now, but I heard it helps to avoid it initially

  • I second the extended release. I had constant stomach issues on the regular pills and it was not a fun time. The extended are much more easier on the tummy/digestive system.

  • I definitely recommend the extended release. I took the regular stuff for a couple of years and still continued to at least occasionally have horrible, out of the blue, don't see it coming explosive diarrhea :(. Sorry so graphic but it's true. On XR I initially had bad headaches and constipation but that resolved quickly.

  • Did your ER prescribe the metformin, or regular physician? I asked my ER about it and he told me he doesn't think I need it .... but honestly if it has positive effects with IVF and maintaining a pregnancy IDK why he wouldn't give it to me.

    ME - 31, DX with dual hydrosalpinx (2012) , confirmed again in 2014, starting IVF 1st Cycle Dec 2014

    Bloodwork done 8/8/2014 LH - 2.8, FSH 6.1

    DH - 31, testing scheduled for 8/22/2014, 60-70% motility, no problems found

  • My RE prescribed it. But we also tested my glucose etc. and my levels are high. I'm in pre-diabetes range, so I assume it has to do with that. Also my egg quality on my first IVF was poor so I think maybe this and a couple other things will help that be better. I'm not really sure how it all works together, lol.

  • I asked my RE if I should be on it. I have PCOS and Insulin resistance. RE said it doesn't really helps, but also it doesn't hurt either. I told him it helped with conceiving the last the two times.He said I am a good candidate for Metformin, and started on extended release ones. I used to be on 1000 mg a day before, but now I have to go upto 1500 mg over the next three weeks.
  • Have you heard of inositol? It's a B vitamin and there is some published research out there that inositol helps with improving egg quality especially with IVF patients, also helps with PCOS by basically doing the same thing as metformin does. I was diagnosed with PCOS in July with my high testosterone and anovulatory cycles. My RE said I didn't need metformin though because I was responding well to my medicated cycles. I am on a 3 month break right now from medicated cycles and also started to take 1800mg inositol last month and this is the first month I have ovulated on my own (confirmed through my temping charts) and currently in my 2WW. I mainly wanted to take inositol to lower my testosterone levels because my hair was falling out. I will be getting my testosterone levels checked this month to see if it's working. Do some research on inositol and see if it's something you can bring up to your RE and try. I know some other ladies who are doing IVF, their REs have them on inosotil.
    Me: 37 DH: 37, married since Sept. 2013. "actively" TTC for a total of 13 months (after 7 months of TTC, took a 1 1/2 year hiatus due to work scheduling and wedding planning. Started up again after our wedding in Sept.) First evaluation by an RE November 2013

    HSG-normal
    OH-tiny polyp found, but RE assured me she doesn't think it's contributing to infertility and doesn't recommend me to remove it because it's so small.

    Hormones all within normal levels
    Ovarian Reserve-RE says "is great" she's not concerned about my egg quantity (which I was until I saw her and results came back good!) 

    04/2012 50mg Clomid=BFN (prescribed by OBGYN, not monitored)
    06/2012 50mg Clomid=BFN (prescribed by OBGYN, not monitored)
    11/2013 100mg Clomid, with HCG trigger=BFN (prescribed by RE, monitored with US (2 dominant follicles) and baseline bloodwork done)
    01/2014 100mg Clomid, with HCG trigger-(prescribed by RE, monitored with US-1 dominant follicle)=BFN
    06/2014 7.5mg Letrozole, HCG trigger (prescribed by RE, monitored with US-2 dominant follicles), IUI #1 on 7/3/14=BFN
    07/2014 7.5mg Letrozole, benched for IUI#2 this cycle, because I ovulated on my own before my follicular US. Progesterone suppositories=BFN Diagnosed with PCOS this month too. Testosterone levels high at 76. =( 
    08/2014 7.5mg Letrozole, HCG trigger with 19mm follicle. IUI#2 on 8/31 cancelled last minute due to family emergency. 
    09/2014 Taking a 3 month break starting this month. If not pregnant in 3 months, will start up again with medicated cycles and IUIs. Had all my labs and AFC re-evaluated this month also. RE says nothing much has changed since my eval last November, okay for me to take a little break. =)

I am stopping it, and don't let me scare you, because it has lowered my AFC and lowered my response to meds in my cycles. But with your insulin being high you need it, because insulin levels can cause immature eggs during Ivf and if you have immaure eggs it is worse than having a lower egg count because you can't use the immature eggs.     Thanks! Yeah I believe in my case it should (hopefully) help with the egg quality. The stomach issues... It has been interesting!! Lol. I'm very cautious about what I am eating, because I have been fine overall but I guess some foods I'm just gonna have to stay away from!

  • I really wish you the best with the stomach issues. I am certain it will get better overtime. You really do need it with insulin issues! Ill be stalking to see how things go for you.

  • I had the stomach issues (like, urgent panic time) when I went up to 1500/day.  I'm on extended release and things leveled out after being on 1500 for about a month.  I also take inositol. My RE says taking both is fine.

    I have been doing okay but am getting worried about the increases... I've read in a few places that people had trouble at the 1500 mark! Was it related to specific foods you are or was it just your body adjusting in general?

  • jencity9 it was just in general.  I actually gave up gluten, sugar and dairy a little bit after starting Met, so I wasn't even eating any of the foods that others have mentioned trigger bathroom disasters.  (I've read that some find eating carb heavy stuff can cause issues.)  

    It definitely gets a lot better after the adjustment period. Also, I take my dose at night, which seems to have helped as well.
