Am I the only one who seems to still not know the difference between my baby's cries?? She doesn't seem to distinguish between hunger diaper and being tired. I feel like I don't have this mothering ability and should by now as she will be 12 weeks on Wednesday!
No, I'm not 100% sure all the time either. I just go through the possibilities to figure it out. If it's been 3 hours, I feed her, if it's been 2 hours I change her and try to put her down for a nap. She's 9 weeks and It's all trial and error here
I can only tell the difference when it's a need-to-burp-or-toot cry. This is my 2nd kid, and all the other cries sound the same to me. Don't feel bad. Like others have said, they get easier to decode when they're on a predictable routine.

I couldn't for a long time, but now she has two very distinctive cries - she "moos" for hunger and she has a sobby, low-pitched cry when she's exhausted.

No. We go through "the list" of things it could be. If it's close to eating I will just nurse him and start from there. Otherwise we change diaper and rock and if all else fails and he's just fussy I put him in his crib and turn on his mobile and he becomes mesmerized for a few minutes.
I only really know the tired cry. It's kind if dramatic and drawn out. It usually ends in some lazy babble. Otherwise she is usually crying because she's hungry or gassy. She doesn't cry when she's wet or poopy. Then there's the mystery cry that we just can't figure out.
Max cries and makes a sort of Nnnn sound when he's hungry. He also moves his mouth in a sucking form. He also moves his head around to try and get the food.
When he's just cranky or in a dirty diaper, or gassy he screams bloody murder. And the cry sounds more like ahhhhhhhh!!! He also gets really red and very still like he doesn't want to be touched a whole lot.
When he's sleepy, he cries very softly. No real distinguished cry though. Just very soft til be falls asleep.
Pricilla Dunstan was on Oprah and spoke about how to recognize different baby sounds. I feel like this has helped us distinguish what Spencer needs or wants.
