Using pumped milk

So..I am new to all this. Lo is 9 days old. Please help clarify this:
1. What do I do to take refrigerated milk out with me for the day ( lets say to go shopping)? Can it just thaw and last 6 hours or do I havw to have ice packs?
2. Can you defrost frozen milk and use or does it have to defrost in the refrigerator?
3. What do you do with milk left in the bottle after a feeding? Save it or toss it?
4. What is the longest room temp milk can stay out before refrigeration?
5. What is the longest refrigerated milk can stay in the refrigerator before being frozen?
6. How long must freshly pumped milk be refrigerated before combining it with other refrigerated milk?
7. If anyone has the Dr Brown bottle warmer...do you warm up the bottle with nipple on it?
