WWYD- Jumping to IVF

I had my first IUI and am in the dreaded 2WW but am not hopeful as my DH had a poor post wash count so I am already trying to come up with our plan for next cycle.  I am not sure what my RE's thoughts are with his counts or what our next steps should be but I am thinking I want to jump straight to IVF and heres why.  I currently have IF coverage (20k lifetime max) but my company's policy will be changing next year (starting Jan) and I fear I will lose coverage.  Although I live in a IF mandate state,their policy is self funded which means they don't have to follow the mandate.  The reason I think they may drop it is because we were recently purchased and they are looking to cut benefits ALOT.  I work in HR and know they are cutting certain coverage but not sure what and will not know of our new coverage until the end of October. 

 So here's my question... would you jump into IVF after only trying IUI once to take as much advantage of insurance coverage while I have it or try 1 more IUI and wait until I know our coverage before making a decision ?  Also, IVF ladies, what is the typical timeframe to go though the whole process?  I am trying to get an idea of what is involved and if we would even have enough time to go through the process this year.  I know this is all putting the cart before the horse but I am a planner (it keeps me sane to know we have a plan) and I keep running all scenerios through my head.

  • @pintobean39 & @nariadreaming.  Thank you ladies.  I guess what is making it tough to make the decision is I am not emotionally ready for IVF but also don't want to lose out of the coverage because I know how lucky I am to have any coverage. Also, they don't cover meds which I know is a few thousand dollars which we will have to come up with quick.  We have been saving but I thought we were going to have a few more months until we would have to make that decision.

    Official diagnosis: Unexplained IF. I am 32. I have low ovarian reserve (low AMH), and poor egg quality. I've also been diagnosed with mild glandular developmental arrest (lining problems, detected with EFT).

    We are using open ID donor sperm. IUIs #1-7=BFN. IVF September 2014 antagonist protocol, 8R,5M,3F, 5 day transfer of 1 morula = BFN. IVF#2 planned for January 2015 (antagonist protocol + HGH).

  • We got a personal loan from a credit union for our IVF so maybe that's an option for the meds part? I totally know what you mean about trying to get things down before the end of the year and insurance changes. Have you spoken to the RE about this yet?

    2008 - 2010: dragging my feet out of fear and procrastination
    October 2010: first consultation with RE, dx PCOS and fibroids (DH slightly low count/motility)
    Oct. 2010 - Dec. 2012:  In DENIAL! avoided the issue because I was scared of surgery
    January 2013: returned to RE, fibroids grew significantly
    February 2013: second HSG, fibroids pushed on tubes which blocked them somewhat
    March 2013: MRI to determine what type of surgery may be necessary

    July 29, 2013: fibroids (5) removed via robotic laparascopy
    August 2013 - Nov 2013 : benched due to recent surgery

    IUI #1, Dec. 24, 2013, BFN 
    IUI #2, Jan. 25, 2014, BFN
    IUI #3, Feb. 25, 2014  BFN
    IUI #4 canceled due to lack of response to letrozole
    IUI #4.1 April 28, 2014, BFN

    May 16, 2014: wtf consult, start prepping for IVF in June and add injects for one last IUI in the meantime
    IUI #5 started letrozole and bravelle but canceled after HSG led to new diagnosis

    May 21, 2014: third HSG, tubes blocked, one at the beginning, one hydrosalpinx??
    June 11, 2014: consult, approved to move on to IVF because the hydro is not completely blocked therefore allowing fluid to move through slowly rather than backwards
    IVF #1 August 8, 2014 - 3dt of 2 embryos, BFN
    September 17, 2014 - 4th HSG, the right tube is very patent (open!!) dye went straight through this time. Weird!
    October 2, 2014 - started metformin treatment
    November 14, 2014 - blood work, brought A1C down from 5.8 to 5.5

    November 26, 2014 - RE finally back from vaca and reviewed my chart, no more IVFs for rest of calendar year
    December 1, 2014 - Right after Thanksgiving, I called a new clinic and got in right away! Plan for IVF
    December 17, 2014 - ER! 29 retrieved (!!), 16 mature, all 16 fertilized (ICSI)

    IVF #2 December 20, 2014 - 3dt of 3 embryos, BFN

    We are done with treatment unsuccessfully. :(

  • I did it, and I'd do it again in a heart beat!

    My DH had far less than ideal numbers for IUI, we did one IUI and I was scheduling an IVF cycle as I was walking out of the office. I can't see your siggy, since I'm mobile, so I don't know your history, but I would definitely consider it given the coverage could be ending!

  • I would definitely jump on it! In the long run a few thousand dollars compared to $10k+ a cycle is small beans. Could you open a no credit card with no interest for x months incentive to cover meds if you don't have the cash right now?

  • Thank you again ladies.  It is alot to think about but I think I am going to talk it over with my RE if this cycle ends in a BFN.  During our initial consultation with her she did mention not wanting to waste too much time on the "non high tech" methods since I do have coverage, she would rather spend it on something with higher chances.

    @iputketchuponketchup I am very open with my parents about IF and they have offered to help but I would rather not ask them.  We do have a credit card that we use for emergencies only and I wouldn't think twice about racking up some charges!  You are so right... where there is a will there is a way.

    TTC: Since January 2013 
    DX: PCOS. Severe Endometriosis, Unicornuate Uterus w/only left tube and left ovary, Pedunculated fibroid (on the outside of uterus) and Anovulation. All conditions diagnosed 8/13
    TX: Metformin
    DH DX: MFI - low morphology, low motility
    Ultrasound shows both kidneys in spite of UU. 
    HSG showed clear tube on the left side. 
    Lap Surgery performed 1/9/14 to remove fibroid and endo (Stage 3)
    • IUI# 1 June 2014 started 100 mg of Clomid - 7dpiui Progesterone: 13: BFN
    • IUI#2  July 2014 started 100 mg of Clomid - 7dpiui Progesterone: 5.75: BFN
    • Natural Cycle - so shocked to be in 2WW - 7dpo Progesterone: 15.5: BFN
    • Working with new RE starting injectables in late August.
    • IUI #3 August 2014  w/ Menopur: BFN
    • Finally ovulating on my own!!
    Waiting to start IVF hopefully

  • One of my biggest regrets is not jumping into IVF when our RE recommended it. We choose to keep trying IUIs, mainly because I was too scared and emotionally not ready for IVF. In hindsight, I wish I had figured out how to overcome my fears and move to IVF.

    If the RE recommends it and you have coverage, do it. Don't wait.  Good luck to you!

    January 2011 - December 2012: 4 cycles w/Clomid; 9 IUIs w/Follistim & Menopur;  two IVF cycles converted to IUI (poor response/dominant follicle) 

    August 2014 IVF (antagonist protocol: Menopur, Follistim, Ganirelix, dexamethasone): 13 eggs, 9 mature, 8 fertilized w/ICSI; 0 frozen, 3 transferred day 5 = BFN

    October 2014 IVF (micro-lupron protocol: mircrolupron, Menopur, Follistim, dexamethasone): 8 eggs, 4 mature, 3 fertilized w/ICSI; 3 transferred day 3 = BFP! Beta 1=121; Beta 2=287; Beta 3 =678. Miscarriage @ 8w2d. :(
  • If I were told I would be given 20k to use towards IVF but had only 3 months to use it, I'd jump at the chance in a heartbeat.

  • I would definitely jump on it, both because of the $20k limit and the possibility of your coverage ending.

    As for the timing, I'd say it'll be tight, but potentially possible. It kind of depends how quickly you can get in for an IVF consult somewhere and where you are in your menstrual cycle at that time. If you could get in for a consult during the 2ww, that would be ideal. If not, call and ask to talk to a nurse. Explain the situation and see if it might make sense for you to start BCPs when you get your next period so you're ready to go when they are (you often have to take BCPs anyways) and see if the nurse can at least schedule you for any tests or classes you need while you wait for your consult. (For example, they might want you to do CD3 tests and if your consult is on CD 10, you could have to wait a few weeks to do it, potentially delaying you.

    Lastly, ask how they bill the cycle. Like, if it is billed as a package, what has to be done in that month for it to be billed in that month. Is it just the baseline appointment? ER & ET? If it's not billed as a package, you'd have to be done by the end of the year, but if it is billed as a package, you might just have to have your baseline or ER or something done by the end of the year, which would also buy you some time.

  • I would move to ivf if I were in your shoes. We decided to move to ivf after 2 failed iui's with poor post wash counts. I know what you mean about not being emotionally ready, but I have to say that after I made the decision I came around pretty quickly. Good luck making your decision!

    January 2010 SA results: Count 16 million, Motility 40%, Morphology 2%
    January 2010- Surprise BFP! DS born 10/1/2010 :)
    January 2013 TTC #2
    September 2013 Repeat SA: Count= 1.7 million, Motility= 24%, Morphology= 2%
    November 6th 1st Appointment with RE: diagnosed with severe MFI
    Testing to try to determine a cause & possible treatment for MFI
    CD 3 blood work for me. RE does not want to repeat my HSG/lap at this point,
    but may want to before moving forward with any fertility treatments.
    After seeing the uro, DH is currently taking lots of supplements and clomid to try to boost his count. We will have a repeat SA in February to see if it works.
    Follow up SA numbers are: Count= 4 million, Motility= 40%, Morphology= 1%
    Uro wants us to have another follow up SA 5/9 to see if we see further improvement than we are back to the RE to make a game plan.

    SA 5/9/2014 Count: 12 Million, Motility: 60%, and Morphology 2%. We will be doing iui #1 in late June

    IUI #1 6/28 clomid + ovidrel, post wash count 3 million total sperm= BFN

    IUI # 2 7/21 clomid+ ovidrel. post wash count 900,000 total sperm= BFN

    IVF planned for early November- cancelled due to cyst

    December IVF #1- 22 eggs, 20 mature,16 fertilized

    12/9 Transferred 1 4AA Blast, 6 frosties
