Triggering a cyst?

Has anyone ever had to do this?

This cycle I was supposed to start Femara again on CD3 but when I went in, there was a cyst on my ovary. I couldn't start the meds yet because they don't want me to feed the cyst. I went back a few days later and it was still there and apparently giving off hormones so my doc had me do the hcg trigger on CD6 and then do timed intercourse so that I "don't waste this cycle". I'm a little confused though, could the cyst be considered a follicle that could release an egg? I feel like with a pretty good sized cyst that the cycle is already wasted so I don't see how it could be saved. Or is the trigger supposed to try to help get rid of the cyst? Any experience with this?

Re: Triggering a cyst?

  • Ummm... That is so weird. If they're sure it's a cyst it most likely has no follie in it, but maybe they're thinking you could potentially build up a follie somewhere else with that trigger? The thing is.. Triggering on CD6, any follie would still be so tiny on CD8 when you are Oing, so I don't see how much good it would do to TI, but hey, stranger things have happened, so you might as well HIO anyway.

I've never heard of triggering a cyst anyway, depending on the size (I get them a lot) they will either cycle me anyway, or just cancel the cycle and let them resolve on their own.

  • Thank you for your response @Mrs_Castillo!

Yes, I thought it was strange since it was so early in my cycle and I really haven't found any info about it at all online.

You say they cycle you anyway... they aren't worried about 'feeding the cyst'? That's why my doc didn't want me to take the meds so it was either trigger or call the cycle a wash. So basically I feel like I shelled out $65 for a trigger that may have been doomed anyway. It's so confusing.

I don't think that has ever been an issue, I know I had 2 cysts the beg of my last IVF cycle and by the time retrieval came along I had 11 mature follies, at that point they didn't say anything about the cysts so maybe they were gone by then? Or maybe they just weren't an issue?

I got them more frequently on Clomid cycles as well, and can't recall ever being canceled then either.

Hopefully everything will be resolved for you and you can cycle next month.

  • That sounds odd to me. I've had follies not release and turn into cysts but I've not heard of what your talking about. Still, if it works and makes the cyst go away I guess it's good. Keep us posted. Now I'm curious.  Good luck :)

I've triggered cysts a couple of times.  Both times I got cysts going off of BCPs.  My RE had me trigger so that the cyst would hopefully release.  Basically, it tricks your body into believing you ovulate (or possibly ovulating a not so good egg -- either old because it stuck around from a previous cycle or immature because it is new and hasn't had enough time to grow to maturity).  Both times I got AF about 2 weeks after trigger and everything looked great on a CD 2 baseline.

    Good luck!

ETA - was your cyst producing estrogen?  Mine were both times.

Now I'm curious. STL I see you have endo. Were your cysts that were producing estrogen endo cysts?

I'm interested because my RE recently saw what he thinks is a 2cm endometrioma on one of my ovaries. Trying to learn as much as I can without going crazy by falling down the Google rabbit hole.

No, they did not believe my cysts were tied to endo.  They believe my cysts were more due to being DOR and my body trying to cycle through BCPs.  A friend of mine had endometriomas.  She had them drained and something injected into them that was supposed to suppress them from returning.  However, I think at least one kept returning.  I hope your doctor is able to figure out a good plan for you, BookishMomma!

Yes, it was found to be producing estrogen. I think that's why they didn't want me to start the Femara.

So did your cyst go away after the trigger? Was it gone by the start of your next cycle?

  • The second time I triggered a cyst it caused it to collapse and I started a new cycle 14/15 days after the trigger (even though I did the trigger on CD 2). The first time I had to trigger a cyst was when TTCing #1 and I don't remember it as well.
