Anyone have an individual STD policy they can reccomend?

I'm self employed and looking for a STD policy that pays well for a c-section. I'm wondering if anyone has a recommendation for something that is not directly tied to the employer. Also, if you are self-employed, how do they verify your income for the percentage of income pay out? Is it just an average of the last year or something? TIA!
