Abnormal?

I am 21 weeks with di-di twins and I don't go back to the OB till 24 weeks and he didn't schedule an ultrasound and they haven't checked my cervix. I already had a healthy pregnancy and no real stand out signs of anything wrong I think just some extra fear that carrying an extra baby brings. Anybody heard of a doctor not checking your cervix with twins? Or has anybody had cervix issues with no warning signs or something I should look out for that I might not know of?

My dr said they could measure it on ultrasound and wouldnt do manual checks until 28 weeks. I was worried about this too, but no issues and still closed when I was induced at 35w.

  • When was the last time you had an ultrasound? Any cervix checking at this point would be via ultrasound and I would also be kind of worried if they hadn't checked in awhile. I think my MFM's protocol was every 3 weeks. Mine had started to shorten around 20 weeks (2cm) and once that was discovered I was getting checked once a week. After 24 weeks it's normal for your cervix to start shortening so my MFM stopped checking since cervical length does not correlate to PTL risk after that point. I think it's worth a conversation with your OB to make sure they are tracking this, especially if your OB is not a MFM.

  • I get induced at 38w tomorrow... I've had a grand total of 3 manual cervical checks. They've done them pretty often on ultrasound (as well as checking the placentas) but they never made a big deal out if it unless you ask them or something is wrong.
  • I had one cervical check via u/s at my anatomy scan at 19w. I asked at my next u/s and was told they aren't done unless there's a reason.

I had a stint in the hospital for very mild pre-e at 32w and was checked manually then as a baseline. I was 1.5cm dilated, 70% effaced, -1 station.

My water broke spontaneously at 34w. I dilated to 6cm within an hour and delivered via c/s due to baby a's prolapsed cord (he was also breech).

I was told that without a compelling reason, the less times someone gets all up in there the better bc of the risk of infection.

  • Mine checked it at my anatomy scan at 18 weeks via ultrasound and again one more time at another ultrasound.  I never had manual ones done, but I also delivered at 30 weeks.

  • My doc started checking mine every 2 weeks at 20 weeks, then weekly after either 28 or 30.  They checked w/ultrasound when I had scans, manually every other time.  I delivered at 34, a surprise, when the week before my cervix was still at 3mm long.  :/

  • My doc checked at 20w with a vaginal u/s and since he saw some funneling, manually checked as well.  I went in again 10 days later and he did the same thing.  No changes.

I am going in every 2 weeks from now (now 25w) and I believe he will keep checking and doing the same thing.  If he didn't see any issues, I'm guessing he wouldn't be checking every appointment.

  • All of mine have been at 2 week intervals with my OB and the MFM checks me every 4 weeks. It is always via transvaginal u/s. Manual checks are a no-no (for any pregnancy) at my OB's office, but I know they are all different. Still, I would refuse a manual check unless in active labor. I am 28 weeks and still 3+cm.

I do think it is abnormal that haven't checked you between 20-24 weeks. That is really a pivotal time.
