AW: First RE Appt

I figured it would be easier to update this rather than posting separate updates in all of my check-ins :-)  

Today was my first RE appt... I thought it might not go very well b/c this am when DH called to see if they had gotten his SA the nurse said they had and the Doc already ordered a new one b/c yours is a year old.  DH was immediately skeptical of any doctor that might order a test when he hadn't met with us... So, DH wasn't feeling very good about that.  Queue the actual appt... they got us in pretty close to on time and walked us back.  The nurse/office staff person immediately gave us some paperwork and a pitch for genetics testing... DH felt not so great about it again.

Doc came in and took our history and then ordered an ultrasound and internal exam... Ultrasound went ok - they thought it looked like I had ovulated because I had a collapsed follicle (this is even though I have no temp spike).  The ultrasound tech believes based on the ultrasound that my endometriosis has returned.  UGH.  But they don't feel like it is hindering my chances of staying pregnant.  Doc was concerned with my late ovulation.  Side note DH was in the room for the ultrasound and exam... poor guy, I think he nearly fell over when he saw the forceps or whatever it is that they use... 

Doc ultimately decided to treat us with Clomid (50mcg) with monitoring and a HCG trigger shot at about Day 13.  He's hoping this will cause us to O earlier which will help.  He said if we don't get pregnant in 2 cycles he would twist our arms to do IUI and possible switch to Femara to try something different... He also said that if we go more than 6 cycles he'd recommend IVF.  

We are still waiting on the results of my Karyotyping to come back through my OBGYN and we will share it with the RE for sure.  

Overall we feel pretty good about the plan and like the doc.  Both DH and I didn't like the initial staff person that came back in all the time to talk about money and recommendations and whatever else... If we had just talked to the doctor it would have been fine.

Oddly enough, the doc didn't order the SA for DH today... lol.

  • So glad the appointment left you in a good place.  I've heard from a number of people that sometimes you don't get the same warm fuzzies with RE offices- not sure what the experience has been here but as long as you're getting good advice and hopefully a baby to take home, sometimes the warm fuzzies don't matter so much :)  Good luck!

    I'm so glad you were able to speak with the RE. I can relate to not always getting the fuzziest feelings from the RE but I have found the more you speak with them, voice your concerns/wishes/questions the more trust that can form and the easier this tough road can be. Big ((hugs)) to you and YH as you start this next path on this journey.

    Repeat c-scar surgery June 2015
    2nd and last IVF cycle August 2015, stimmed for 12 days, 2 egg retrieved, both mature and both fertilized.  Transferred both 8-cell embryos on Day 3, beta 9/5/15 = BFFN
    MOVING TO ADOPTION!

    I'm glad your plan is in place! My experience with clomid is limited as well, but like @meladoriestar‌ says it's usually based on follie size. My first round I went In for an US on cd 12 and my follies were only 17 and 19, so dr told me to wait 2 days and then to trigger. She assumed my follies at CD 14 would be 19 and 21. (They say they grow 1-3mm every two days?) This time I was In on CD 11 with two giant follies @ 20&22mm! So she triggered me then. My dr says I can plan to O 40 hrs after the trigger. Just a little insight to the clomid & trigger route! :) and also, I'm on 100 mg of clomid CD 3-7. FX that this works for you :)!!!

    BFP #1 August 24, 2013! MMC discovered Oct. 3, 2013, D&C Oct. 4, 2013
    BFP #2 December 17, 2013! MMC discovered Jan. 28, 2014, D&C Jan. 30, 2014
    Testing done: male with complete Trisomy 16, not hereditary. Tested me for clotting disorders, all normal.
    Feb. 2014 all clear again to TTC!  Will start progesterone supplementation with the next BFP just in case.       Oct. 2014 more testing just because, thyroid and autoimmune panels = normal.  Diagnostic U/S = no abnormalities.  Will keep trying for 3-6 more months, doctor still optimistic!

    BFP #3 December 11, 2014!  Beta #1 14DPO = 122.4 Beta #2 17DPO = 296.8 Please stick little one and be our Rainbow!

            Perfect little heartbeat of 156 seen 1/7/15 :)

    Thanks for the info everyone...@rslh10 and @melasoriestar - they will start monitoring at cd13 and then tell me when to trigger.

    @snegde no formal diagnosis really... But I do know I have endometriosis, PCOS, and hashimoto's... No new diagnosis...

    ETA- I don't know what to do about the trigger. Etc because I usually O around cd20 and I'm supposed to be out of town on 18&19 of my cycle stays on track this time...

    Glad the appt ended up going well. So DH is on board and likes the dr too? Poor guy, mh was pretty shocked the first time he was in the room for an exam.

  • @Km380‌ yes, DH is as in board as he could be... He likes out OB better but he's on board... He doesn't understand how IUI would help (and neither do I really) but overall he's good... It just felt a little businessy....

  • I'm glad to hear that despite a few setbacks in the beginning the appt went well. I don't know much about RE appts, but it sounds like you have a good plan and I'm thankful for that. ((Hugs))

  • Glad you like the doctor. Boo to the staff though. I had a similar situation wit the RE I was seeing and probably will not go back to him.... Not because of him but his staff. If i feel like I need to go back to an RE I will be looking for a different one. I am glad you have a plan of action now and hope that it helps you get your rainbow. Lots of :love:: and ::hugs::

  • @MrsG80inTN‌ I'm glad in the end you liked the RE. I'm sorry that the rest of the office were pushing other things on you. I hope that's just a one time sales deal.
    I went for genetic counseling when I first started ttc that wasn't too bad and insurance covered it. It was just going over family history to determine if there could be possible issues. I haven't done genetic testing, although we thought about it. The price trumped our want to know.
    I too have been/ am on clomid and a trigger shot. Please know that there are lots of us on here that can support you and ease you through that process. GL and FX!

    I'm glad it went well overall and you have a plan in place!  Regarding the trigger, hopefully you'll be ready to go earlier based on your monitoring, but if not, you should be able to go on CD 17 and they should be able to tell you if you should trigger while you're away...I would just give them a heads up at your earlier monitoring appointments.  It is based on both follicle size and E2 levels.  FX the Clomid gets you ready to go earlier so you don't have to worry about the travel at all!

  • I'm glad the appointment went well overall, even if your experience with the staff wasn't great. I get a little of that with the lady who does "financial counseling" at my RE's office, too. It's clear she's more of a sales rep/bill collector than counselor. 

    And the push for the genetic testing just felt weird to us, too. Weird because it was that lady pushing it. The cost was affordable, but the doctor hadn't even seen us yet when she tried to sell it, and there was just something strange about the way it was presented. My guess was there's some big kickback to the RE for referring patients, but who knows? I may be paranoid, but it felt strange.

    I don't have any experience to offer regarding Clomid or late ovulation, but the plan you described sounds solid, and I'm glad you have a clear path to move forward. FX that this leads you to your rainbow sooner rather than later.
