Twins! Empty gestational sacs?

Hi, first time mom. Need some hopeful stories and prayers. At my 7 week abdominal ultrasound they found 2 separate gestational sacs measuring 6w5d and 6w2d. No yolk sac no fetal pole in either. I was greeted with bad news from the midwife preparing me for the worst (blighted ovum and eventual miscarriage) but scheduling me for another u/s one week later. I requested they do some blood draws in the mean time to monitor my hcg levels (I was surprised they didn't offer this and I had to request it myself?). After 48 hours my hcg went from 15,000ish to 17,000ish which they said was good but not as much as they'd like to see, especially with twins. I still feel pregnant, no spotting/cramping, mildly sore boobs, fatigued, and slightly increasing queasiness. So next Thursday I am scheduled for a transvaginal u/s (I am also surprised they didn't do a transvaginal u/s in the first place?) and now I am playing the waiting game. Anyone with a similar situation?

Re: Twins! Empty gestational sacs?

  • I always had transvaginal scans done up to 11-14 weeks bc of prior losses. Surprised they didn't do that. My numbers would double every several days they drew blood. If you are not bleeding or spotting it is good signs though. Good luck
  • I wouldn't worry until you get transvaginal ultrasound. I can't believe they didn't do one.... No wonder they couldn't see anything in there!! I've never had anything but transvaginal ultrasounds until after the 12 week when your uterus gets pushed far enough out of the pelvic bone to get a decent image. The fact that your hcg levels were still climbing I think is a good sign, they don't usually double anymore once the numbers get that high...... Keep thinking the good thoughts until your next ultrasound. And keep us posted!!! :)

  • I had my first u/s around the same time and they saw two sacs; one with a visible yolk sac and fetal pole, the second did not have anything visible. My doctor told me it was probably a vanishing twin and would be gone the next time I went.  Well when we went a month later there were two healthy babies.  Because they didn't go transvaginal I wouldn't worry to much, especially because you haven't had any symptoms.  Wishing you the best of luck, take care of yourself and hope you get great news at your next appointment!

  • I agree with PPs, it is way too early for an abdominal u/s. FWIW, I measured off by a week in the beginning. I went in for a transvaginal scan (because we were IVF) at 6 weeks and all we saw were two completley empty sacs measuring 5 weeks. Went back a week later and we had tiny flickers for hb, but still measuring only 6 weeks, so still 7 days behind. Development can be a stark difference of seeing nothing and seeing a hb in a matter of days at this point. I will keep you in my T&Ps for a better scan next week!

  • Agreeing with everyone else!  I didn't have an abdominal u/s until 12 weeks, and they weren't sure if they'd be able to see everything they needed to see then.

We found out twins at my 8 week appointment with a transvaginal ultrasound.  Anytime my docs have needed a better view, they'll do a quick transvaginal ultrasound.  I'd try not to worry too much until your next appointment; however, I know that's much easier said than done!  Also 6 weeks is still very early to tell anything for sure.
