Is this a normal treatment plan?

DH and I have been ttc for a year now. I don't get a period so the problem is that I'm not ovulating. I've had this problem my whole life really… got my period very late (16) and it was never normal so I went on BC at 17 and was on it for 11 years. RE did ultrasound and I at least have plenty of eggs. My uterine lining is very thin. They started me on a round of premarin for 10 days followed by provera for 7 days. I am anxiously awaiting blood results, too.

The appt was such a blur, I didn't ask enough questions. I was told I probably have a "lazy pituitary" gland which is causing me not to produce enough FSH so I don't ovulate. But this is only a starting hypothesis…  I was surprised that my RE didn't discuss PCOS or Clomid just yet. He is waiting for my blood results and then may have me do the HSG test. 

Is this a normal first step with an RE? I'm afraid maybe we aren't being aggressive enough. I have underlying medical condition (that doesn't affect fertility) but will make me a high risk pregnancy if/when i ever do conceive. It's just so much information.. I'm not sure if I'm going crazy here

  • It does sound pretty normal to me.  You need to get a period so they can do the rest of the testing, like an ultrasound and bloodwork that needs to be done on CD3, and then you need an HSG to make sure your tubes aren't blocked. What bloodwork did he do already?  It may not be useful, because most of the labs needs to be done on CD3 or 7DPO to be of any use.

  • If you don't have all of your blood work results, and you haven't had your HSG, or your husband hasn't had a SA yet, then you're still in the testing phase. Most people don't move forward with true treatment until this is all complete.

    I also don't ovulate or have a period on my own. I had to take Provera to start my "period" to do testing, then wait a month and take Provera again to start another "period" before starting Clomid. Do you think your doctor is doing something like that? Once you have all of your test results in and you've had your follow-up appointment, I'd think you could start some more aggressive treatment, but you might want to just ask to clarify.

  • Thanks ladies. This is my second round of provera and first with premarin so I'm glad to know its standard.... Keeping my fingers crossed for good testing.
  • You should have your prolactin checked. After I went off birth control in my early 20s, I never naturally got a period again. As it turns out, I have a prolactinoma, which is a small tumor on the pituitary gland. It's totally a livable condition it doesn't really do any harm, except my body produces too much prolactin, which both suppresses estrogen and stops the menstrual cycle. They're also quite common, and often go undiagnosed. If you do have a prolactin issue, you probably will not be able to get pregnant without medication to control the prolactin. The good news is, most of these issues are very easily controlled with a small amount of medication. It could be your pituitary just fine, but it's just a simple blood test. Good luck!

  • Sounds totally normal to me. We just wrapped up our dx phase. Sounds very similar. The ladies here are awesome when it comes to advice and confirming things. Good luck!

  • A lot of REs won't do Clomid until you've had an HSG, because it's pointless to make all of those eggs if your tubes are blocked and nothing can get from point A to point B.  The diagnosis phase of all of this sucks, because it's hard to sit and wait and feel like you're not doing anything.  FX that your stay is short and sweet.
