Ovulation-OPK and chart

took clomid 150mg on cd 10-15. I had ovarian drilling two months ago. I have been using digital opk and haven't gotten a positive yet. Yesterday and today I got positive line opks. I'm so confused. Here is the picture of my chart and opk . Any advice? Are the digitals not as good as they say? Hoping for a bfp this month!

  • Same question as @ky29. Clomid at any level is dangerous if taken without monitoring...but 150mg is SUPER unsafe.

  • We are not doing a follicle check right now because we are 2+ years into ttc. We have done that before but after the ovarian drilling the doctor advised we retry clomid with our reg obgyn. We are not interested In IVF so the RE passed us along. Before the ovarian drilling I did 250mg of clomid twice in one month without ovulating so I'm kind of shocked to see a positive opk for the first ever.
  • We also are not eligible for the trigger shot because I always have several small follicles and the dr worries that I might be a john and Kate +8 scenario if I take the trigger shot. So I have to agree to harvesting eggs before the shot.

    Just because you are 2+ years in, doesn't mean you can skip out on the monitoring - it's still dangerous, even if you didn't ovulate at a higher dose. You should request monitoring. And just because you don't trigger, doesn't mean you won't release multiple eggs. Not trying to be mean, just saying you should be monitored.

    If the clomid causes you have multiple follicles you could still end up with HOM even without the trigger shot. Without the proper monitoring you have no idea how you're responding. There's also the increased risk for cysts....and you won't know it the clomid is thinning your lining....it all just makes me very nervous for you.

  • I'm not sure what ovarian drilling is. Is seeing a new RE an option? From what I understand the OPK just tells you when you have a surge, not necessarily when you ovulate. It's been years since I charted... I'm sorry I guess I'm not sure what you're asking.. ?

    2008: HSG (normal), couple rounds of clomid through gyno
    2008 - 2010: dragging my feet out of fear and procrastination
    October 2010: first consultation with RE, dx PCOS and fibroids (DH slightly low count/motility)
    Oct. 2010 - Dec. 2012:  In DENIAL! avoided the issue because I was scared of surgery
    January 2013: returned to RE, fibroids grew significantly
    February 2013: second HSG, fibroids pushed on tubes which blocked them somewhat
    March 2013: MRI to determine what type of surgery may be necessary

    July 29, 2013: fibroids (5) removed via robotic laparascopy
    August 2013 - Nov 2013 : benched due to recent surgery

    IUI #1, Dec. 24, 2013, BFN 
    IUI #2, Jan. 25, 2014, BFN
    IUI #3, Feb. 25, 2014  BFN
    IUI #4 canceled due to lack of response to letrozole
    IUI #4.1 April 28, 2014, BFN

    May 16, 2014: wtf consult, start prepping for IVF in June and add injects for one last IUI in the meantime
    IUI #5 started letrozole and bravelle but canceled after HSG led to new diagnosis

    May 21, 2014: third HSG, tubes blocked, one at the beginning, one hydrosalpinx??
    June 11, 2014: consult, approved to move on to IVF because the hydro is not completely blocked therefore allowing fluid to move through slowly rather than backwards
    IVF #1 August 8, 2014 - 3dt of 2 embryos, BFN
    September 17, 2014 - 4th HSG, the right tube is very patent (open!!) dye went straight through this time. Weird!
    October 2, 2014 - started metformin treatment
    November 14, 2014 - blood work, brought A1C down from 5.8 to 5.5

    November 26, 2014 - RE finally back from vaca and reviewed my chart, no more IVFs for rest of calendar year
    December 1, 2014 - Right after Thanksgiving, I called a new clinic and got in right away! Plan for IVF
    December 17, 2014 - ER! 29 retrieved (!!), 16 mature, all 16 fertilized (ICSI)

    IVF #2 December 20, 2014 - 3dt of 3 embryos, BFN

    We are done with treatment unsuccessfully. :(

  • I think the purpose of the ovarian drilling is to remove some of that harder ovarian material with PCOS that stimulates production of androgens, and also just to stimulate the ovaries in general?

    Your temps do look like you've already O'ed, and it is possible for an LH surge to remain detectable even a day or two after O has occured. That said, it's not really common, and Clomid can mess with temps, so there's no way to know for sure unless you get monitoring.

    It's a little odd for an RE not to want to work with you unless you move on to IVF...would your OBGYN be willing to monitor you each cycle? There's just no way to know how many mature follicles are likely to O, and you'll want to know if this dose stops working. Plus, it's always possible to miss the LH surge with predictor kits based on unlucky timing of the surge compared to your test time(s). 

    So, in sum, I wish you the best of luck for regular and healthy ovulation after your ovarian drilling procedure! Hopefully your OBGYN will be willing to monitor more closely both for safety (risk of cysts, multiples) and to ease your mind as you time babymaking. :) Good luck!

  • I am now at my desktop and can explain in more detail instead of typing from my phone:) Thanks for the responses. I know my situation is confusing as it is still confusing to me as well. Prior to ovarian drilling I tried several cycles of clomid and femara with my OB and then my RE. When I was going to the OB they helped confirm ovulation by bloodwork. I never ovulated. When I started at my RE I would do the clomid go in for a follicle check and bloodwork. When they called I would restart the clomid again if no mature follicle was seen. As I said earlier I did 250 mg of clomid and when I went for a check they put me on it for another week. I always have several small follicles so after the tries of clomid my RE refused to trigger and do IUI because he was scared I would get pregnant with 6 or 7 babies so he told me our next step would be IVF. We did the ovarian drilling two months ago with my regular OB because insurance paid for it through her but not my RE. I got a regular cycle about 24 days after the drilling. After a full cycle she let me try the clomid again because she feels it could work. This is where I am currently. I havent really tracked temperature to much but she encouraged me to try it again, but I dont know much about the numerical signs. I have never in my life had a positive ovulation so when I saw the positive posted above I was so excited because now would be the time I ovulate. When I used the digital to confirm it gave me an error. When I tested after church this afternoon digital was negative and line opk was once again very positive. So I guess my question was has anyone else ever gotten a confirmed ovulation with a negative digital. I have read online that for some people they dont pick up until post ovulation. I understand that I should probably be monitored better but my OB doesnt really do what my RE would. My IF diagnosis is related to PCOS. I currently take 2000 mg metformin daily. I hope this all makes sense. Thanks once again for the responses and concern. Good luck to you ladies! 
  • I never actually used OPK's digital or not so I can't comment on that but according to your chart it looks like you O'd. I had heard meds make charting/OPK's all wonky though so I'm not sure. But then a nurse recently suggested I use them even though I have been on Letrozole, so I don't know.

    And it's interesting to me that your RE won't let you trigger with small follicles? Have you had any mature ones? I thought those were the only ones to worry about. I have PCOS too so I always have lots of small ones but I believe anything below 10mm is not counted.

    Anyway, sorry if I keep sounding confused, but the chart itself looks good to me!
