Trouble TTC

My first RE appt and some questions

Hi ladies,

Thank you so much for welcoming me here and calming my nerves about my RE appointment! I just got home from it and it was a lot to take in!

First off, my insurance sucks. No treatments are covered and only some of the testing. It's confusing to me but they said we will go over it as it comes. The first cycle which should start in about a week will be a monitoring cycle with cd3 and 7dpo Bloodwork, ultrasound motoring and an hsg all in between there. DH will also get a more extensive SA done too. The RE told me if my remaining tube and my numbers look good we can either try on our own or go straight for a medicated cycle or medicated IUI which he recommends. He said something about 5 days only of clomid but from what I read jumping straight to clomid isn't great, right?

Does all this sound about right to start off? He made it seem like with one tube it will take much longer so he wants to jump into medicated cycles. Any advice ? I would be monitored through every step of the process and he really seems knowledgeable and very caring which made me feel at ease. All in all, it was a lot to absorb but I'm feeling a little better and feeling like I have something to look forward to.

  • Sounds like you have a good plan! Clomid is usually the first med they use and since you'll be monitored it sounds good to me! I would definitely do medicated since we can't control which side the follicles grow on and that'll give you a better chance if having at least one on the side you need. I personally went straight to IUI though I was offered to just do TI. I wanted the best chances possible!

    July 2006: started TTC
    2008: HSG (normal), couple rounds of clomid through gyno
    2008 - 2010: dragging my feet out of fear and procrastination
    October 2010: first consultation with RE, dx PCOS and fibroids (DH slightly low count/motility)
    Oct. 2010 - Dec. 2012:  In DENIAL! avoided the issue because I was scared of surgery
    January 2013: returned to RE, fibroids grew significantly
    February 2013: second HSG, fibroids pushed on tubes which blocked them somewhat
    March 2013: MRI to determine what type of surgery may be necessary

    July 29, 2013: fibroids (5) removed via robotic laparascopy
    August 2013 - Nov 2013 : benched due to recent surgery

    IUI #1, Dec. 24, 2013, BFN 
    IUI #2, Jan. 25, 2014, BFN
    IUI #3, Feb. 25, 2014  BFN
    IUI #4 canceled due to lack of response to letrozole
    IUI #4.1 April 28, 2014, BFN

    May 16, 2014: wtf consult, start prepping for IVF in June and add injects for one last IUI in the meantime
    IUI #5 started letrozole and bravelle but canceled after HSG led to new diagnosis

    May 21, 2014: third HSG, tubes blocked, one at the beginning, one hydrosalpinx??
    June 11, 2014: consult, approved to move on to IVF because the hydro is not completely blocked therefore allowing fluid to move through slowly rather than backwards
    IVF #1 August 8, 2014 - 3dt of 2 embryos, BFN
    September 17, 2014 - 4th HSG, the right tube is very patent (open!!) dye went straight through this time. Weird!
    October 2, 2014 - started metformin treatment
    November 14, 2014 - blood work, brought A1C down from 5.8 to 5.5

    November 26, 2014 - RE finally back from vaca and reviewed my chart, no more IVFs for rest of calendar year
    December 1, 2014 - Right after Thanksgiving, I called a new clinic and got in right away! Plan for IVF
    December 17, 2014 - ER! 29 retrieved (!!), 16 mature, all 16 fertilized (ICSI)

    IVF #2 December 20, 2014 - 3dt of 3 embryos, BFN

    We are done with treatment unsuccessfully. :(

    @apresmoi , I think that's why I was very overwhelmed. My obgyn made it sound like no big deal that I had one tube (probably cause she's the one who took my other one lol) and the RE kind of set the stage that it could take a lot longer because of it. Jumping into medicated cycles makes sense I guess I just wasn't expecting it but then again who would expect any of this right ? Thank you for your advice, it definitely helps to hear from people in my situation.

    I think one tube can be "not a big deal" for some people and a huge deal for other people if you have other IF issues that go along with it. For example, my father lost one of his testicles as a child, but my parents had absolutely no issues conceiving when they wanted. If he had been having trouble though, the missing testicle would be more of a concern. 

    So that's probably why your doctor though it was no biggie, but your RE is more concerned about what only one tube means in conjunction with IF treatment.

    Sounds like my appointment as well! I only have one tube as well (the right one is hydro and I'm having it removed soon) My re didn't feel that the hsg was necessary, as I had been been pregnant and mc twice. So she agreed to let me skip out of it. But I did opt to go on clomid as I wanted to increase the likelyhood of having an egg on my left side each month.
    All the doctors I have spoke with were not overly concerned about one tube (a couple said it might take a little longer) but if I understand correctly your body starts to compensate a little bit for the missing tube. Not completely, but a little. Mother Nature is cool that way.
  • Sounds like a great plan! Sorry that your insurance coverage isn't what you hoped. Hopefully everything will go well and you won't end up needed a ton of OOP treatments!!
