We used a postpartum doula after ours were born and it was a lifesaver. She specialized in multiple births and had all kinds of great logistical tips on feeding 2 at once, bathing, swaddling, etc. I would highly recommend this if you can swing it. We got a gift certificate from friends at our baby shower.
Personally I would not recommend a birth doula with twins - even when things are relatively uncomplicated, it's kind of like giving birth in grand central station - so many people in the room and we had awesome support from the nurses and dr.'s. Plus many hospitals don't allow more than 1 support person in the delivery room with twins, assuming you will be in an OR.
