painful letdown - extended BFing

DD is 22 months, we have successfully BFed without many issues. Lately, when she nurses, I get a painful feeling on the other side during letdown. Sometimes, when she first latches on, as well. I never had this before. I do not have any plugged ducts or pores (had my fair share, so I know what they are like).
Any ideas?
