I once had an older male patient (fresh off a brain injury) and we were working on appropriate comments, or at least quelling inappropriate comments. So we were generating responses to social situations:

Me: Your wife says, "I'm so tired, I could sleep for a week!" What could you say back to her?

Patient: "If you're going to sleep for a week, when are we going to have sex?"

Me: Try that one again.

Patient: "Can I have sex with you while you're sleeping?"

Me: No, please try that again. What's something that would be appropriate to say to your wife?

::devious old man smile::

Patient: "Could you sleep with your mouth open?"

It was. I just about died laughing and then composed myself and said that it was completely unprofessional for me to laugh like that.  He smiled again and said, "But that was a good one, wasn't it?" So it was a fun look at the unfiltered male brain, albeit 70+ years old. His wife was less amused.
