Doing my taxes - anybody know how Short Term Disability is handled?

I am about to start my taxes and wondered how STD plays a role.  Was I supposed to get some tax form from the company who paid out my STD? 

 

I still giggle at the abbreviation for short term disability.

Mine was just factored in to my regular w2. Most of the time it is unless you were paid for an exceptional amount of time.

My medical leave was factored into my W2, also.

Since I paid 100% for my STD I was sent a tax form.  Does your employer pay a portion?

They paid for the first week and then the following 5 weeks came from Prudential who handled our STD.

Mine came from another company, too.  Hope they took taxes out of yours becuase they didn't take them out of mine and no one mentioned it at the time.  Goodbye, tax refund!
