I started the transitions at when baby turned 11 months (she will be 1 in a week). I started by using 1 oz milk and the rest formula (or breast milk). Each week I increased the milk an ounce. Now we are at 3 oz of milk. However, I only do it at her nighttime bottle. I don't think there is a right or wrong way to do it. Every baby and family is different. I know lots of people that have done it differently.
We transitioned slowly by mixing it with breastmilk little by little over 3-4 weeks. Prior to that she took 12oz of breastmilk a day at daycare, and nursed 2-3 times at home. Now she has wcm with breakfast, lunch, and dinner (and water with snacks)... and still nurses 2-3 times per day. I would bet she's drinking 8-12oz of wcm per day, but I don't measure.
I'm mixing 1/2 WCM and 1/2 BM in each bottle until the BM in the freezer runs out. She's also still nursing 2x/day, so I know she's getting plenty even when she rejects the bottle.
After 22 months TTC and the loss of our identical twins at 10w6d, we're excited about our rainbow baby!

My daughter was 11 1/2 mo and I would do a regular cup of formula (bc she used a sippy cup at 10 mo and no bottles) then I would give her a little bit of whole milk and she liked it so I gradually switched from formula to wm like 2 cups of formula and 2 cups of milk for a couple of days then traded one cup of formula for whole milk. So she would get 1 cup of formula and 3 cups of whole milk then took out formula completely but she only gets 3 cups of milk a day. Her ped says no more than 24 oz of whole milk and no less than 16, different peds tell u different but 3 cups which is 24 oz is good for her.
We switched straight to whole milk at his 1st bday. My pediatrician said he needed around 2-2 1/2 cups a day....he took the AM sippy great but afternoon and evening not so well for the first month. I just supplemented with yogurt until he started drinking it at all mealtimes. He loved yogurt so that is always easy! Good luck!!
