help! DS is hitting constantly!

What do we do with time outs??  He will hit several times during a diaper change for instance. Do we give him a time out once the diaper is back on? He will hit when we are playing and having fun. Do we give him a time out for every single hit or do we ignore some and do time outs for the hard one? I'm so lost in this area. When we firmly tell him to not hit, sometimes he will cry, sometimes he will just look the other way. He hits so often, if we give him a time out every single time, it could be about 40 time outs in a day.

Just stop him or hold his hand and say "no hit, be gentle" and then show him gentle while smiling. Do it every single time, I swear it'll work.

DS is 16 months today. Sometimes he hits out of the blue when he's not even mad or frustrated.
