Mine just says - regular &/or increasing intensity contractions.
When I went into labor with DS, they were not regular by any means, but the intensity was growing to unbearable. We decided to go to the hospital after I finally had a couple that were 4 mins apart. We had been timing them for a few hours at home. They would be 8 mins, then 5, then 12, then 4, then 6, then 10, then 8, then 4, etc... they were all over the board for me. They did hold me in triage when I got there for about 3 hours before officially admitting me to make sure I was progressing. I believe once I was there, they were better at being closer together more consistently.
My midwife didn't discuss when to come in until my 38 week appointment (so last week). She told me 4-1-1, unless my water breaks, in which case I need to go ahead and come in for monitoring.
For a FTM my midwife wants a call once contractions are 4-5min apart, for STM+ 5-6min apart.
Water breaking isn't a reason to call in right away, I am to call if contractions don't start within a few hours OR if there is meconium in the water. With DS he had meconium when my water broke so I needed to go in ASAP.
I told my OB I wanted to labor as much as I can at home, and she also knows and is supportive of how committed I am to a natural, un-medicated birth. When I asked when I should head in, she confirmed that I live about 25 minutes away, and said, "well, then I'd say head in when you feel the urge to push." I was shocked but relieved to hear that from her...sounds good to me!

Mine said if my water breaks or I have bleeding. She didn't mention the 5-1-1 rule, but said to head to the hospital as soon as I have a few painful contractions. She thinks I going to go fast, 4TM, I hope she's right.
I am supposed to go in at 7 minute contractions for 45 minutes. I am a snowflake though, because I am already 5 cm dilated and 90% effaced and a STM. The doc is actually quite concerned about me making it to L and D on time.
Initially I was told to call my doctor and then immediately head to L&D if (1) my water breaks, or (2) the 5-1-1 rule (cannot talk or walk through contractions).
Due to baby being in breech position and high BP, doctor told me to call immediately when contractions become painful (as they will want to perform a c-section).

I plan on asking this on Thursday when I go in-- and also because there was just another instance of birth-on-highway here recently I'm even more terrified of when I should go in. The traffic here is so sporadic and weird and it makes me nervous! (especially because there really isn't another way to get there, either) I guess we'll see.
