Trouble TTC

HSG question - why where your tubes blocked?

I want to try and summarize this as quickly as possible, so here we go :)  My husband and I are both 30, TTC for about 14 months.  I have regular periods (30-32 days), no major pains or bleeding.  My husband was tested last month and he was completely fine on all four tests.  So, this month, my OBGYN suggested I come in the week before expected ovulation and she wanted to do an ultrasound to see if anything was amiss.  The US showed I was about to ovulate from my right side, but she saw fluid in the left fallopian tube.  So, fast forward to today, AF came and now she wants to order a HSG for the fluid.  (She said you should never see the tube, the fluid was a concern.)

So, I looked it up, and it seems that any blockage is caused by PID, an untreated STD, or endometriosis.  Is this true?  I was tested for STD's when i started at this doctor and all was clear. (I have slept with 2 people my whole life, my hubbie and i have been together for 10 years. So, it wasn't majorly concerned.)  I haven't been "tested" per say for endometriosis, but my periods are light, not painful, and regular.  So, I feel like if it was that or PID, I would have SOME symptoms.

So, rambling aside, has anyone had a HSG and had a blockage (or fluid) and it was from something other than a STD, PID, or endometriosis?  I am freaking myself out that i have some undiagnosed disease I haven't had symptoms for!  Sorry, I know you all know how crazy we can get during this whole journey.

Thanks ladies :)

Re: HSG question - why where your tubes blocked?

  • I am wondering this too as my HSG showed my left tube was blocked. I have no history of STDs/PID so I don't think it could be that. No history of abdominal surgery that could have caused scarring either. I am honestly worried that it could mean I have endometriosis... but trying not to focus on that right now.

So basically I'm no help :) but I am interested in seeing other ladies' opinions.

My RE wasn't sure what was causing mine. We suspected endo, just because the other 2 options didn't fit with my history. It turned out to be none of those things, but we don't know what it was now. All he can tell me is not endo & gone. I can't sit here & say that will happen for you, but it seems like there are other possible causes.

What is your plan going forward?

  • I have a blocked tube as well, but my RE didn't say why. Once a long time ago my family dr's medical student told me my abnormal paps were caused by HPV, but I have no idea if that's true, I did ask the RE if abnormal paps have anything to do with blocked tubes and she said no.

  • Mine was only partially blocked, but it was because I had cysts on the ends of both tubes. I do have endometriosis, but the cysts were unrelated to that - doctor thinks I've had them since birth. I don't think I had any symptoms of the cysts because nothing is different from before I had them removed. Also, they never showed up on any ultrasounds so we didn't even know I had them until I had surgery.

I'm not saying to this to scare you or make you think you have endo because I have no reason to think that you do, but I didn't have many typical symptoms of it either. I had a general pelvic ache most of the month, but my periods were light, regular, and pretty painless too. I was really surprised when my doctor started suspecting endo because I only knew people who had really painful cases.

  • I had a very thick lining which was blocking both tubes.  I had a D&C to "clean things out" and that did the trick.  They tested me for endometrial hyperplasia, which I didn't have (thankfully).  Maybe just another side-effect of PCOS.   I would think that an US would detect the lining things.

Scarring and cysts were two other reasons for blockage.

  • My partner's tubes were blocked and damaged (one complete blockage, one hydrosalpinx) due to SEVERE (stage 4) endometriosis.   This was confirmed via a lap surgery last year.   There are  so many adhesions in there her tubes were blocked closed and tangled, and her ovaries are "kissing" and affixed to the back of her uterus (they should be separate and free-floating).

Both tubes were removed during the lap last year, and we are currently undergoing IVF in an attempt to harvest as many eggs as possible due to the endometriomas she has as well.

Because my partner is queer, and has had very very low-risk of infection for any STIs her whole life, we were confused about the PID aspect as well with regards to her blocked tubes.  In fact, that was what pushed us to ask for the Lap surgery instead of simply blocking off the hydrosalpinx with a coil.  Endo was the only thing that made sense and we wanted it confirmed in order to make some choices.    It was confirmed and her tubes were removed.  That was a hard week for us, and is still hard to grieve...  but we are grateful that we now know what was going on.

I hope you find answers yourself.

  • Thanks for all the advice, I appreciate it! My ob called today to see if I had any questions and I asked her why they would be blocked. She said it could be endometriosis, a previous infection, or just a blockage with unknown origin. So,basically I need to wait until the test next week to see what's going on.  Good luck and baby dust to you all :)
  • My RE said mine probably became blocked after my D&C for my miscarriage because I had some scarring by the openings. But he also said it could be caused from many things including debris from your uterus getting pulled in.
