IF lurker here Today at my monitoring appt, my E2 came back at a little over 3000 (29 follicles between 13 and
18). I'm supposed to stay the course with 150
follistim 1x a day and 20 units of lupron 2x a day. I go back on Sunday
for another bloodwork/ultrasound. Doc says that if my E2 on Sunday is
over 5000, we'll probably do a "freeze all" with any embryos and do a
FET later. Is 5000 really high? Should I be worried?
For those of you who have gone through this- can you do a FET the next cycle? or do you wait?

Secondly, unfortunately there is no way to know if you will get OHSS even with a high e2 level. My first fresh cycle e2 got up to about 4600 at trigger, second was up to 6300, but I was lucky and never had any OHSS either time. I was only dealing with 10 and 15 eggs each time so I don't know if a higher number of follies would help contribute to the OHSS since you would be dealing with more fluid left behind after ER, KWIM? We did go ahead with transfer both times and ended up with c/p's both times as well. I do not think that my e2 had anything to do with those results necessarily though, it appears like it was an implantation issue that was resolved later on.
It sounds like your RE is trying to be cautious which can be a good thing, just try not to get ahead of yourself at this point. If your levels do shoot up too much it's not a bad idea to be proactive and keep an eye out for signs of OHSS, but it's not always the case that you will automatically get it.
Hopefully on Sunday you will have a better idea of what to expect. A lot can happen at this point! I am really hoping that everything is looking good for you on Sunday!

It really is hard not to get ahead of yourself with this! There is just no "right" or "expected" outcome. Every cycle is different. And yes, all you can do is go along with the ride at this point.
You could always ask your RE about checking you for signs of OHSS after ER if you would rather not do a freeze all at this point. Especially if you are being lined up for a 5dt.

FWIW, my e2 was sky high at one point (~11k) a few days before retrieval. We coasted a few days and I'm not sure what it ended up being at ER. They retrieved 35 eggs, 34 of which were mature. I only had very, very minimal OHSS, which surprised the heck out of everyone. In fact my RE was ready to do a freeze all but I ended up being ok to do a fresh transfer.
I've seen girls with half the e2 and follies though get pretty bad OHSS. I really think it depends on your body. I'm also unsure if I just got lucky. Be sure to eat a LOT of protein during this time. Wishing you the very best! Good luck sweets!
My last fresh cycle my e2 was nearly 5000 at trigger and there were well over 20 measurable follies but wound up with 45 retrieved!!!! 33 were mature yikes!!! After ER, I had a mild case of OHSS but my RE wasn't concerned and we did the ET anyways. I wish I had just waited and asked for a freeze all since I didn't think it'd work with all that going on inside me....my FET was was worked for me after all 6 weeks later but I had to go through yet another BFN with my last fresh to get there. Best of luck whatever you and you're RE decide on!!!
