What Did You Do? Delivery Question

edited November 2013 in Multiples
I am now 33 weeks and Baby A has been vertex since 25 weeks. Baby B has been breach for about the same amount of time.

My doctor will deliver A vaginally and attempt to turn B but will also deliver both via c-section should I prefer.

Assuming there is no change in position, why would/did you do?

I would clearly rather have vaginal birth with less recovery time, but I am terrified of increased risk to Baby B.

D & L are here at 34 weeks 4 days by vaginal and breech delivery on 11/19/2013
Two healthy boys weighing 4 lbs 15 ozs and 4 lbs 5 ozs.  Only 6 days in the NICU and getting bigger, stronger and cuter every day!

Re: What Did You Do? Delivery Question

  • I had a c/s since both my guys were breech- and they were delivered at 34w 3d- but after talking to my OB and MFM , I decided that in that situation I would go for the c/s- since my OB did not do breech extractions and I did not want to end up recovering from both a vaginal and a c/s delivery..  I had a really easy recovery from my c/s- up and not in any pain by the time I left the hospital at 5 days- off any type of pain med by a week- and feeling .back to normal totally in 2 weeks.

  • If your OB is comfortable doing a breech extraction, I would definitely attempt a vaginal delivery.  If your OB is going to go straight to c-section if Baby B stays breech, I would opt for a c-section from the start.

  • Same as the PP. I had a doctor who was kind of thrilled to do a breech extraction (and she had many years experience doing them), and it went just fine. They tried a manual inversion but the sucka wouldn't turn. If they wont attempt a breech extraction, I would opt for a c/s.

  • Planned a c section because baby B was transverse. Had some bleeding the week before, and we discovered baby B was vertex. They asked if I wanted to be induced, and I agreed. After baby A was born, baby B flipped and was breech. Drs conferenced and agreed they would do a breech extraction because baby B was ready to come out and my first child was over 10 pounds. My girls were 7.2 and 6.7. All went well. Two pushes and she was here.

I was to nervous to agree to attempt delivering both unless they were both head down, but in the end I am so glad it worked out this way.

It was the strangest 5 min of my life as my doctor held baby b, by her foot, to keep her inside me and discussed c-sec vs breech extraction with the other dr!
  • I was in the exact same boat a couple weeks ago as my baby a is head down and baby b is flipping between breach and transverse. I decided to schedule a csection even though I really wanted a vaginal delivery. I don't want to risk having to recover from vaginal and csection if baby b wasn't able to be delivered breach. I have my csection scheduled for next Thursday, but they said to request an ultrasound beforehand to confirm that baby b is still breach and if he does decide to flip in the next week, I can always deliver vaginally instead of csection. Good luck to you on your decision! It was hard for my to make up my mind!

This! My Baby A was vertex and B flipped back and forth from breech to vertex until the end. My doctor was willing to do a breech extraction as long as Baby B was smaller. Baby B ended up being almost a pound bigger, so I opted for a c/s. I had an u/s right before my c/s prep and if they had both been vertex, we would have induced instead.
^^ this. I would have only tried vaginal if both were head down. Baby B was bigger by a lb so breech extraction would have been out of the question. Both my girls were head down but it was a moot point since I ended up having to have a c/s anyways. They wouldn't induce me bc baby A was in distress.

    I agree with this.

  • This is something we have been discussing after every ultrasound appointment. Baby A for us has always been head down from the beginning. Baby B has flipped between head down and transverse. Now, at 35w 2d, they are both head down. My dr says that's actually the most efficient way for them to be positioned and she feels confident Baby B won't move again. As of right now, we are opting for a vaginal birth. Our Baby B is 2 lbs bigger but we have had the conversation with our OB about my pelvis/body being able to birth a child that big. She does not have any concerns right now.

However, I would not feel confident birthing Baby A vaginally and then doing a c/s for Baby B. If Baby B wasn't in position, I would probably go ahead and do the c/s from the get go. I would not feel comfortable with the risks associated with turning a breech baby.

    Good luck!

  • When I delivered, baby A was head down, baby B breach. I delivered both vaginally and baby B was breach when I delivered. No tears, stitches, etc. and I didn't feel a thing. I did deliver in the operating room as a precaution though since it was twins and second was breach. So, it's possible everything will go great! GL to you. :D

  •  It was tricky to have C in my head an have to switch over to "oh my gosh! Now I actually have to push two people out!?!?!?" So...if you at all want to attempt vaginal...I'd think through/plan for both in your head so there's no huge shock in the end if that makes sense. Hope you get the delivery you want!
This…this is one of the reasons I decided on c/s early on.  When it was time they were both breech anyway, though.
