Success after IF

Low AMH/high FSH - IVF

Hello! I'm from the 2IF board but was looking for some hope and thought I might find it here. I'm 39 and have a 3 year old DD.

Today was my CD3 labs and they came back horrible .09 AMH and 21 FSH. I have a left blocked tube and my right ovary doesn't produce well.

My RE said I could go forward with the stim cycle and see how it goes but doesn't think I will respond well. In Sept, we did Bravelle and Clomid and I produced 2 follicles - 15 and 23 on my left ovary with the blocked tube. So IUI failed. When my AMH was tested in July it was .16, so it's reduced drastically in just 4 months.

The protocol I start tomorrow is 150 menopur and 300 gonal f and then back to the RE on Monday for labs to see if it's working.

Does ANYONE know of success with Low AMH/high FSH and IVF? Also, has anyone had this same protocol?

I'm completely devastated to say the least and just looking for some hope.

  • I'm really sorry for the stress of all of this. My RE didn't want to test my AMH since he knew I needed a high dose of stims anyway and my FSH, while quite high for my age (29 at the time), was not too alarming. But, I also did 150 Menopur and 300 Follistim in my successful cycle. I did a Lupron Stop protocol with a low dose of Lupron that stopped once stims started. Through my previous IUI and IVF cycles, we learned that I have a very thin line of not being suppressed enough/getting over suppressed. My AFC on CD3 was 6, but I ended up getting 17 eggs and 16 embryos. Unfortunately, my embryos tend to tank as they transition into blasts, but our two boys stuck. Anyway, I continued on that dose until a couple of days before trigger when my estrogen levels stuttered for a minute. They bumped me up to a total of 525 IU for the last night.

    I'm so sorry I couldn't be of more help. It was so hard to cope with the thought of going through so much with so little hope. Many hugs and well wishes your way.

  • My AMH was normal during my last IVF cycle, but my FSH was pretty high (especially for my age). I apologize as I don't remember right now what the number was. My official dx is DOR (MFI is a factor as well). I actually did a microdose Lupron flare protocol - 150 Menopur and 450 Gonal-F. I think I stimmed for 13 or 14 days, and we only got 6 eggs at ER (5 mature), but we were able to do a 5dt of 2 blasts, and have 1 frozen blast from that cycle as well. Our twins are almost 6 months old now, so it was definitely a successful cycle :) I wish you the best of luck!

  • I have undetectable amh, low afc and high fsh but I got pregnant with clomid-iui-prog with first and spontaneously this time so doesn't really help you on the ivf issue.

    When I was going to do ivf, my RE recd MDL protocol. I have also heard that antagonist is popular for poor responders, but I think MDL is usually tried first. Are you on any supps? You might ask about baby aspirin, vitamin d and coq10. I was on those when I got pregnant. Also, search high fsh over 40 network 54 on Google. There is a great board with a ton of information. Don't worry if you are not over 40.

    2011: FSH 13.3 & E 99; AMH 0.54 2nd FSH 6.2 E 40's AFC: 8
    BFP from Clomid/IUI ~ Pre-e and IUGR during pregnancy ~ DS born 9/4/12
    Feb./March 2013: AMH less than 0.16 (undectable) and AFC = 4;
    BFP from supps ~ DS#2 due May 2014

    Thank you for responding. It says Antagonist at the top of my IVF schedule. I see from your signature that your FSH was higher in 2011 and lower in 2013 - it improved. Did that change because of your supplements? I'm not on any supplements now. Would it be too late to start being I'm on my 2nd day of injections?

    I asked the nurse about Lupron and she said because my AMH is so low they wouldn't consider using that. She said they have to suppress me on the "back end" rather then in the beginning.

    Well, my most recent FSH was 11 so it really has not gone down.  (None of the readings were on supps.)  I've had it done 4 times: 13 (with elevated E), 6, 6, 11.  It varies from cycle to cycle.  I don't have my most recent FSH in my siggy. I have tried to edit it about 2 dozen times with no luck. :/  But, over the past 2 or so years, my fsh really has not gotten a lot worse...but my AMH and AFC has.   

    My most recent RE prescribed microdose lupron protocol even though I have undetectable AMH so I don't really know...I have heard that sometimes they try antagonist if MDL doesn't work or on DOR+AMA.  IDK...I think it just depends on the clinic.  In doing research, it seems like most clinics are split on whether MDL or antagonist is best for DOR.  I don't know if one is better than the other per se, so I don't think they are wrong to start you on antagonist.
