Those that had Pre-E with HELLP Syndrome with your first

I developed Pre-E with HELLP Syndrome with my twin pregnancy. I'm wondering if for your next pregnancy, were you considered high risk because of it? I really want to have a midwife with our next pregnancy but I'm afraid I'll be considered high risk and not be able to. I am really hoping for a VBAC.

  • Can't help ya, but I'm glad you asked bc I've been wondering the same thing. I had pre-e/HELLP with my first and while were still in NICU and not even considering ttc for awhile, I'm curious.

Yes. You will be considered high risk and you should be followed by MFM. DD was born at 27 weeks due to severe pre-e and HELLP. I met with MFM to discuss what happened and risks for a future pregnancy. You are at higher risk for it if you've already had it. I was told that VBAC would be on the table based on how my C-section went, depending on how a future pregnancy went.

If you haven't met with MFM to discuss what happened and future risks, I really recommend it. It was really helpful in deciding if we would try again.
  • This might be a stupid question, but did you talk to the MFM when you were ready to try again or right away after? Also, had you been followed by a MFM when this happened?
  • @Cst2CstBride Thank you for your response. I never had an MFM with my twin pregnancy and I probably should have. Maybe in Canada its different. How did you get to see an MFM? Does your OB refer you or can you just call up? I have no idea how it works.

  • I was not followed by MFM. Aside from needing IUI to conceive, I had a totally normal pregnancy with no issues. Then I woke up in extreme pain at 27w0d. Soon after I developed a severe headache and started vomiting. The MFM I saw was on-call and made the decision to have an emergency c-section and deliver dd. I thought she was great and was really good with my family too.

My OB had to refer me, but she thought it was a great idea too so she was happy to do it. I waited until about 6 mo after delivery, mostly because I needed time to emotionally recover before I could talk about it. Because it took us 18 months and fertility treatments, we wanted to talk sooner because if we were going to try again, it would have been around 8 months. The only reason I think it is beneficial to have the talk sooner is if it will change your plans.

My meeting resulted in me being dx with 2 clotting disorders and I was given a 50% chance of it happening again, with it being equally likely to happen at 24w as 38w. We decided we weren't comfortable with that risk (although my MFM said that she would take me through another pregnancy) and have moved to adoption. I'm glad we had the talk early so that we could have time to make that decision.
  • Thank you for sharing your story. I guess I have some talking to do. I really want another baby but if I'm only meant to have my 2 baby girls then I'd be ok with that. I know that through reading the pg after a preemie thread that there are women that has had full term pregnancies following a pregnancy with HELLP. I guess its finding out the chances of it happening again that I'd have to find out.

  • You would be considered high risk, I remember clearly being told due to the hellp syndrome any future pregnancies would be high risk. The high risk ob that delivered me told me percentages for chance of issues in a second pregnancy, but I can't recall them other then them being low, and not actually applying to me due to some other medical stuff putting me in a higher risk for preterm delivery this time around.

  • Thank you for asking this question. I had my baby Brooklyn last week after getting pre-e and HELLP at 33 weeks. I couldn't imagine doing this again but would like another baby down the road. The doctors that I've seen while in the hospital have said that I would be high risk next time but not to think that it will be impossible. But I'm in no hurry at this point. I'm still getting used to this new world of NICU.
  • @BostonKisses2 Thanks for the insight. I will talk to my doctor about it. I have to make an apt anyways so I'll ask my GP while I'm there.

  • I'm currently 23 weeks with #2. DS was my HELLP baby with pre-e and was born at 30 weeks. I am high risk and was referred to my MFM by my OB at my first appointment. A VBAC can be on the table depending on your previous csection and your health throughout the pregnancy.

So far my "high-risk" classification means I'm starting appointments closer together at 23ish weeks and some extra blood/lab work. Ohh and some extra ultrasounds with my MFM. I'm sure they might pick up as we approach my previous milestone, but it's really not bad. Plus the classification of high risk is so that even if you do begin to display signs of pre-e and HELLP you will get the attention you need rather than being ignored.
