Halp!!! Letter of Resignation ...

So, I informally told my boss of my intention to resign from my position, but I still need to tender my resignation via chain of command.

I have never done this, and really don't know what to say, or the professional way to say what amounts to, "I quit."

Any suggestions?

My answer depends on if you like them or not. If it was my job, I'd like it to say "I quit this Bitch", but that may not be appropriate for yours.

DH will still be working there, so I even if I didn't like them, I would still have to be professional. As it is, I really do care about my boss and some of my coworkers.

The slime slut cunt executive assistant whose perma-yeast infection prevented her from wearing pants like the rest of us can eat shit all day and twice on Sunday, but everyone else I have to be nice to.

In my dream world my letter of resignation would go something like this:
Dear Hypocritial Fuck Faces,
You and your shit ass company can go fuck yourselves.
I am done with your shit.
Fuck you,
Love me.

In reality, I would be much more polite and try my damnedest not to burn any bridges because you never know what the future might hold and you may need them for a reference or something at some point.

The purpose of this notice is to tender my resignation effective X date. I appreciate all I have learned here blah blah blah and wish you well on future endeavors.

I am leaving for lots of reasons. The glossy reason I am giving anyone who asks is that I need to refocus my attention on my family. Wow, that sounds so unambitous, but really, in the end, nothing is more important than them, and I have been neglectful and resentful of them because I am too tied up with work.

Once I left a job and agonized over the letter. It was a great job but it wasn't a good fit for me.

I wrote a beautiful resignation letter than took me an hour to get the wording right. My boss read the first line, put the letter in her garbage and said "see ya".

Just keep it short and sweet.  My last day is such and such, thanks for the opportunity, I plan to focus on my family for a while.

With respect and regret, I must tender my resignation. My last day will be Friday, October 11, 2013. This was not an easy decision to come to, but it was a necessary one. I feel like a member of a family, not just a employee, of the blank job name, and I am deeply grateful for the opportunity I have been given, as well as the experience I gained while employed here.

Thanks, I don't think anyone can get upset with it. Like I said, normally I wouldn't worry so much, but DH hopes to be Chief here one day, and I have to be so, so careful there is no backlash from my actions.
