Nanny Interview-what to ask and pay question?

I am interviewing a nanny tonight for part time care. If this particular woman works out, she will be watching my 7 month old 2 days a week and another family with 2 kids 3 days a week.  Any suggestions what I should ask her and what requirements I should set? Also I mentioned this on an earlier post but the other family pays her $16/hr for 2 kids full time right now. I am thinking for 1 child $12-13 is fair....she is also going to be with us 1-2 hours longer each day than she is with the current family.
