We use them in the NICU for babies with reflux, who need to be elevated after feeds. But they are hooked up to cardiac monitors, and they aren't just placed on there like the baby in the pictures. We give them boundaries so the baby won't roll off of it.
That picture, in my opinion, looks pretty unsafe!! There are other safer options you can use for babies with reflux issues.
Please don't buy this! MAYBE you'll need it if you find out your baby has an actual medical condition and a doctor recommends it. But otherwise you are buying useless and unsafe equipment.

NICU doctors do not recommend any inserts to home cribs. They sometimes recommend literally propping the legs on the head side of the crib or pack n play up a few inches to relieve reflux. But that is in combination with appropriate medicine, no bumpers, etc.
After market "recline" sleepers of any kind, including rock n plays, are not recommended by our hospital's NICU due to positional sleep apnea concerns.
