How do you document?

I was wondering what system everyone was using to document missed visits ect.

Old school calendar?
Something online? screenshots of texts?

    DH records every phone call. We are in a state where it is legal to record as long as one party knows about it. BM and DH use our family wizard, so he does journals as well as making sure everything is documented on the calendar. He tries to communicate mainly on OFW as phone calls with BM used to be over an hour each time with call after call back to him if he'd hang up. It would be anger to crying to guilt-trip, etc and he just got fed up with it and said no more, he was only going to communicate with her by OFW and SS by phone. OFW is a pretty good site.

    We screenshot all issues. We don't have issues with either party not taking their time, BM just likes to get into pissing matches sometimes.
