Doctor concerns

I'm 14 weeks with mono/di twins. I'm also the research queen so I've tried to inform myself as much as possible. Here's my concern, my doctor doesn't seem very attentive. I won't have another ultrasound until 20 weeks (even though from
what I've read, sharing a placenta requires more attention, plus he can't even give me 100% on the two sacs). I feel frustrated, he is the head of the obstetrics dept at one of our hospitals so I would think he's very qualified. I just don't feel
As though my little ones are getting the observation they need. Am I overreacting??

Re: Doctor concerns

  • Try and go see a MFM. I am almost 20 weeks with mo/di twins and they started doing ultrasounds every two weeks as soon as mo/di was confirmed. You are higher risk because of the babies sharing a placenta and you should be monitored more. From what my doctors are saying 6 weeks is way too long to wait until another ultrasound.

  • I had the NT scan at 13 weeks and then the anatomy scan at 20 weeks. My twins are di/di, but I believe they don't typical monitor closer unless there is a concern. I say if you prefer having closer monitoring, you should call around and find out what the routine care is for mo/di twins from practice to practice. Because if the risk for TTTS, I would also prefer closer monitoring.

  • What is a mfm?
  • MFM=maternal fetal medicine, typically a perinatologist sp?

I have di/di and my OB has insisted she do a quick u/s at EVERY appointment as she can not be 100% certain she's hearing both heartbeats with just the doppler. I would absolutely be concerned about your dr if I were you. I also see and MFM and their office does all the in depth u/s and growth scans-I typically see them every 3-4 weeks.

  • You are absolutely not overreacting. I believe it is standard for mo-dis to start monitoring at least every other week for TTTS starting at 16 weeks. I switched to seeing a MFM exclusively at 18 weeks because my ob wasn't taking the situation seriously enough. You need to advocate for yourself and your babies.

  • Go to an mfm! I had the same concerns about my OB who denied me an ultrasound until I finally demanded one (went from 18w to almost 27w without one) and not to scare you but I was admitted to the hospital after both babies had diminished cord flow and A was iugr. If I hadn't listened to my gut we never would have known. Follow your instincts and go see an mfm. They will monitor you and your babies closely and are more qualified with high risk pg than your OB. • I have an appt Wednesday, should I just mention to him how I feel and why I'm concerned? If he doesn't give me an adequate response then switch? I'm not sure (Medicaid) covers a specialist or if we even have one in Beaumont. I'll have to check. Or could I request he refer me to one?
  • I would get online and see if there is a specialist in your area so you are armed with some information. Then explain your concerns and if they aren't supportive then time to make a change.

If you haven't even brought your concerns up yet with your doctor, then by all means, that's the place to start.  And you've also never asked your doctor about coordinating care with a MFM?  Definitely start there, and see what he/she says.  These are things you can ask your doctor at any appointment -- you don't need to passively wait for your doctor to suggest things or agree with every recommendation he/she makes.  Do your own research, evaluate your comfort level, and ask lots of questions about your doctor's recommendations if you are uncertain.

  • Im 25 weeks with mo/di. I was referred to a mfm as soon as my ob found out they were twins. I see my ob every 2 weeks and my mfm once a week. I have a shortened cervix so I go a little more than most probably but you definitely should see a mfm. I also get an ultrasound at every appointment rather its my regular ob or my mfm.
  • Texas! Yes ma'am! I was hoping there was one closer but can't find one. Once a month to Houston isn't bad though.
  • I am 15 weeks with mono di and saw a mfm at 10 weeks. I went back for NT scan at 12 weeks and now starting at 16 weeks will see them every 2 weeks for an ultrasound .

I have the most AMAZING MFM doctor in Houston (actually Webster). His name is Dr. Rowe. I saw him today for the 2nd time. My girls are di/di and he's seeing me once every 3 weeks. Definitely request to see a MFM.
