Weird Appointment

Most imprortantly, everything looks good with the babies. Then, as we were about to leave, MFM asked if we'd discussed time frame for delivery. Not really, we've been focusing on keeping them in. So he said that with mono-dis, they generally like to deliver in the 32-34 week range. I answered that I thought it was more like 36-37 weeks (which by the way is what I discussed months ago with the other doc in the practice). He did say that if everything continues to look good, he's okay with leaving them for longer but I was shocked that he suggested that they should be delivered so early without an imminent threat. We always preferred this doc to the other but now I'm not so sure.

Has anyone else been told that it is better to deliver so early with mono-dis?

  • I haven't heard that about mo-di twins either.  I am having mo-di also but my doctor said as long as everything looks ok, he'll keep them in there as long as they want to be and wouldn't induce until 38 weeks.

  • Seems a little crazy to me, I'd obviously like to avoid NICU time if at all possible. But, it would be awful if something went wrong that could have been prevented by delivering early.

  • That's very early. I know that some (most?) doctors won't stop labor after 34 weeks (with twins), but that's certainly not the same as purposely delivering them that early!

  • I am having mo/di twins and my doctor said she won't let me got past 37 but 32-34 seems really really.

  • My MFM said 37 weeks for mono-di's, 38 weeks for di-di.  Maybe he was thinking mo-mo's?  32-24 weeks is triplets maybe, but that is way early for an otherwise uncomplicated mono-di pregnancy.  If you are being monitored routinely, they can catch things that may happen, like deteriorating placenta's etc that would dictate an early delivery.

    TTC since May 2006. After 3 failed Clomid cycles, 2 failed Injectibles/IUIs, 2 failed IVFs and 1 failed FET, we moved on to adoption!

  • My doc has me scheduled for 37 weeks with my mono di twins. Earlier on purpose was never an option. That is way too early to start labor on purpose!

  • Never heard of elective delivery of mo/di that early.  I know mo/mo's are usually delivered around that time because of the high risk.  The research I've read suggests 36-37 weeks is ideal for mo/di delivery since the risk of issues rises after that point.  My MFM would only let me go to 34 weeks because they had complications but they ended up needing to delivery much before that.
  • I have had some complications, so perhaps that is where this came from. Baby B was IUGR but technically not by definition as of Friday and we've had cord issues. All has been looking better the last couple of weeks though.

    With what we've been through I feel so lucky to have the luxury of even discussing 32 vs 36 weeks!

    That could be why. I had an iugr and poor blood flow twin. MFM said the risks of keeping them inside vs. outside were outweighed at 34 weeks. But if things stay steady and improving for you, hopefully they will let you go farther with close monitoring. I'd just be prepared for the possibility.

  • 37wks was the latest my MFM was comfortable with for mono-di twins. They expected me to go earlier with the growth discordance and GD we had but they ended up letting me go to 37 once things improved.

  • I agree with PPs - maybe there's something specific in your case, but that seems awfully aggressive. They're letting my momo pair with a triplet go to 32, maybe a bit beyond. That all tends to be very specific to the cord entanglement issue though. 32-34 is generally what I've read is standard for momos, not mo/di. I've also learned though that at an mfm practice every specialist is likely to have very different perspectives. This one could be the outlier. Wishing you and your little ones the best!
  • Thanks for all of the replies. Provided things look okay, I'm going to fight to keep them in til 35-36 weeks. I still need to go ahead and get that hospital bag packed just in case though!
