XP: RE Apppintment Update (long)

I've been posting a lot in various places about this appointment coming up, so I figured I would just cross-post an update so I don't have to write about it in several different check ins. :)

We met with my RE today for the first time since my second loss. They had received the results from my karyotyping test which were fortunately completely normal. They are also going to do a full RPL, so they drew some of the blood for that today. My RE wants to run Day 3 hormones again so we'll finish the RPL blood draws then. She also mentioned doing a hysteroscopy as a possibility if nothing else seems to be wrong. I've had an HSG and a saline ultrasound, and she said it is unlikely she will find anything, but it's worth it to me to rule out uterine issues as a possible cause. My midwife told me there was no fetal tissue to test from the D&C, but my RE had me sign off on a request to send the pathology report to her so she can make sure we have all the information. The only other possibility is having our cryobank run genetic testing on our sperm donor. Another RE at my clinic has had success with that, but a friend of mine who used the same cryobank did not, so I'm not banking (no pun intended) on it.

Odds are everything is going to come back normal and my losses will just be unexplained. I asked my RE if I could just be making more aneuploid embryos than normal and she said yes -- that's my own personal self-diagnosis at this point. She said about 50% is normal for someone my age (35), but I could be up at 70%. At this point 75% of my embryos have expired, either in the lab or in my uterus, so I'd like to believe that means the last 4 are perfect! But I know it doesn't work that way. Because of my history and because I now only have embryos frozen in pairs, my RE thinks at this point we should transfer 2. We've been very pro-SET so it makes me a bit nervous, but I think it's time to bite the bullet. If we have twins, we have twins. It's not ideal but it's better than no kids, and we wanted two eventually anyway.

I feel good about the appointment. I do think that getting additional testing and the hysteroscopy will mean I'll have to delay until October instead of September, which is frustrating, but it's worth it to rule out as many factors as possible.   • @ball.and.chain - I am glad to hear that you feel good about the appointment, and that there are some options for additional testing you can do to perhaps get some answers... and even if not, to feel that you have explored all possible avenues for potential answers. I can only imagine how frustrating it must be, but it sounds like you have a good outlook. I am willing to bet that the 4 embryos that you have left are in fact perfect! :)

  • Thanks for the update! I've been thinking of you since this morning. I forget, does your cryobank have record of other live births from your sperm donor? I'm sure you've already checked on that.

    Yep -- I knew he had reported pregnancies when we picked him, but I called to ask about live births when we had our second loss.  If we did another fresh cycle we would pick a new donor just to start fresh, but since we have those embies in the freezer, it seems to make more sense to try them first.

  • I love how thorough you and your RE are being and am glad you have a good batch of next steps! I had to have a hysteroscopy (standard for my RE after a d&e I guess) and it was a new/interesting view of things. I hope they find nothing and it gives you lots of confidence to move forward! As for the twins, I think you're making a great decision with all the factors you mention. Best get working on some more names...i seem to recall a post where you and S could only agree on one per gender! FX for you!!!!

    Ha ha -- until proven otherwise, I am only willing to entertain the idea of B/G twins to avoid that issue!  G/G might work out OK, B/B will be torture (naming, not raising them).

  • It seems like you have a very solid plan in place, and I'm happy to hear it. I'm wishing you the best and I hope all the answers come quickly and you get your family!!

    • DW and I have been tracking, preparing, getting medical testing since January 2013.
    • First Cycle: Unmedicated ICI w/ Donor Sperm 08/02/13: BFN
    • Second Cycle: Unmedicated ICI w/ Donor Sperm 09/11/13, 09/13/13, 09/15/13: BFN
    • Third Cycle: Unmedicated ICI w/ Donor Sperm: 10/13/13, 10/15/13, 10/17/13, 10/21/13: BFN
    • January 2014: Sonohysterogram shows excellent lining & tubes have no blockages
    • Fourth Cycle:  Monitored clomid cycle  w/ ICI's at home: 1/24/14 and 1/25/14. Ovulation verified:  BFN
    • Fifth Cycle: 02/2014 Femara 5mg with ovidrel trigger CD14: BFN
    • Taking a few months off to evaluate if we want to keep trying
