Need advice - Leaving job

Hi,

I am currently on maternity leave and have decided to leave work to be a SAHM.  I need advice, though, on how to tell my job that I won't be returning.  Keep in mind, I don't have a traditional office job.  I work out of my home and am on the road everyday. Should I tell my boss via email/call/both?

Also, I know, traditionally, you give two weeks notice, however, I'm afraid if I tell my boss during my leave, I will lose the last week of leave which is fully paid/full benefits. And I don't have any childcare set up so I can't return for two weeks after my leave. Has anyone else had this situation? Is it horrible to tell them your first day back that you won't be returning? I don't have any plans on returning to this company although I may need them as a reference in the future.
