As w/ any decorating, find one thing you fall in love w/ and decorate around that. Often, for nurseries, it's the bedding. For us, it's a print of the Annapolis Yacht Club where we were married. We thought it would be cool to put it in the boys' room and from there came our nautical theme.
When I first started thinking about the nursery, I thought of the different color combos that might work and then came up with different designs and patterns using photoshop. When I found the color combo I drew the walls to scale in photoshop and worked out the spacing for the verticle stripes. I went through five or six (to scale) designs before making the final decision.
