Hcg increases but no heart beat??

Hi ladies,
So I would be 8 weeks now pregnant now based on my ovulation. I went for my first ultrasound last Friday and the dr saw only a gest sac. My hcg was 29000 so I had another ultrasound this Monday and the dr saw a fetal pole with no heart beat around 5 weeks 6 days. The sac was measured around 1.3cm. My hcg on that day was 44000. The dr said the fetal might stop developing since there's no heart beat. Since my levels are high and increasing, I will have another ultrasound next Monday. Anyone has similar experiences? Thank you!

  • So much that I have read claims that you might not even hear a HB until @ 10 weeks. If you were just shy of 6 weeks, wouldn't there be a huge chance of not hearing it? This seems to go against a lot of the reading I have been doing. But, I am not a doctor...

  • You won't pick it up on a doppler until 10 weeks, but you'll see it on an ultrasound by 5-6 weeks.

    Is it possible you got your ovulation date wrong and are just a bit earlier than you thought?

    I haven't experienced this, but that's only because I don't get to have tests done at all until much later.

  • Just wanted to share my experience and hopefully some peace of mind:) I went for u/s at 5 weeks and we saw no fetal pole just a sac. Went again at 5 weeks 5 days and saw a fetal pole no HB. Went again at 6 weeks 4 days and we saw a HB. From what the tech told me sometimes the HB can't be seen until after 6 weeks. Lots of good vibes for you momma. Hang in there your numbers look great.
  • It's not unusual to not see a HB at 5w6d.  I remember with my first pregnancy, they were surprised to see one before 6 weeks.  Good luck next week.

  • I had an us tues measured 6weeks 1 day and could not hear the heartbeat. We could see it moving though so I go back in two weeks and check again to make sure on track. I should be almost 8 weeks according to lmp so maybe dating is off a little for you too. Try not to worry! Good luck next week! ((Hugs))
  • I think she might mean she didn't "see a heartbeat" b/c even I know no u/s I have ever had can you "hear" heartbeat...I have only heard the h/b with a doppler.  I went for a u/s at 5w4days with this baby and only saw a sac and a yolk then went 9 days later and saw a strong hb at 140..going tomorrow again fora  u/s and I still get so nervous every appt.  HTH and good luck
  • @MrsO11/24/2006 just FYI you can very much hear and see HB on ultrasound. The Doppler done on the ultrasound is measured and thats how the HR is determined. Sometimes the tech might have the sound off and that might be why you haven't heard it in the past.☺ you should definitely ask your tech to let you hear it tomorrow:)
  • Not what you would like to hear, but my first pregnancy was a partial molar pregnancy. You have high hcg levels but the embryo is really a type of cyst. I hope this is not the case for you. As the others say, it is very common to not see a Hb at 6 weeks or less. Sending you good juju!

  • Loss mentioned.... when I had my MC last year, I had an US at 6wk 1 day. Doc saw yolk sac and the fetal pole was measurable at .39cm (3.90mm) crl. Unfortunately there was no heartbeat and I miscarried two days later....however, I know some women who didn't see the heartbeat at first who then went on to have healthy pregnancies.

  • You should NEVER hear the embryonic/fetal heartbeat during the 1st trimester. It is against AIUM guidelines and ALARA principle, they regulate ultrasound protocols. The sonographer should only use M mode, which is measuring motion to get the heart rate. So many old school sonographers still do it even though we've been told it's a big no no. So please don't ask and get upset when your sonographer cannot produce the heartbeat sound. I personally won't even use it during the 2nd trimester unless I'm ruling out a complication. Other sonographers will do it briefly for the parents to hear but I will not. The tiny heart is just too fragile in the 1st trimester for the extra power to be used on it to create the sound by spectral Doppler imaging.

    It is extremely common to not see the flicker of the heartbeat at 5 weeks. Sometimes even at 6 weeks. It's probably just too early. 44,000 is right in line with a 6 weeks gestation. I would not worry about a molar at all. The molar pregnancies I've seen have had extremely high hcg levels that were completely out of the normal range.

    So are you saying if a sonographer did this for us, there was risk to the baby? Just to be clear....

  • @lalala2004 I will say it is outlined by the AIUM, which we are supposed to be following, to only use M mode. I do not want to step on any fellow sonographer toes but I personally do not ever use spectral Doppler. It has been highly debated in the past few years. Some of the older techs out there are set in their ways. But it was beaten into our heads very hard in school and out in the field that only M mode should be used. I personally was yelled at by a supervisor for accidentally hitting the spectral Doppler button. Even though I immediately turned it off she still let me have it. I haven't made that mistake again.

  • Thank you so much for clarifying @autumnlynn21 . I do cardiac U/S so have no idea what OB guidelines are but with my pregnancies the techs have always used Doppler so I assumed it was ok to use. So sorry for any misinformation given.
  • @JSandP1120 A fellow sonographer awesome! Echo is like a whole other language to me. I want to take the vascular RVT board but the Edelman study material is brutal. Definitely have a lot of respect for you guys!

  • @autumnlynne1221 awesome to hear another sonographer on the board☺. I'm strictly echo only, so I have no clue about general but always wanted to cross train☺. Edelman is tough but he has the best study material out there. His review course is great , you should definitely check it out if you decide to get the RVT board.:)
