Endometriosis before IVF

Hi All,

We're about to start IVF with unexplained infertility. Our RE (Dr. Kosasa, 40+ years of experience) would like to start with a laparoscopy for endometriosis. My wife has strong pain in the first 2 days of AF but other than that no symptoms. I understand many women who are over 35 might have endo without symptoms other than infertility. Our Doctor suggests that laparoscopy increases the success rate of IVF so that's the way to go. We don't want to take the risks of a surgery unless it is really needed. We asked around and learnt that some REs don't do laparoscopy before IVF anymore because its effectiveness is not established. When I asked about this to my doctor, he said most REs didn't know how to do laparoscopy that's why they prefer not recomending it.  

It appears that protocols for endo treatment keeps changing. I saw some posts from 2008. I wanted to hear about recent experiences. Probably what I will hear here will be a reflection of how REs are still arguing whether laparoscopic treatment of endo increases the IVF success rate or not but we still wanted to get your thoughts. Have you gone through laparoscopy? Was it easy to recover? Can we start IVF right after lap surgery?

  • I know I am replying to myself. Just wanted to share our experience in case it is useful for others. 

    We had the laparoscopy with our REs recommendation. On the day of surgery, we learnt that he was planning to do curettage in case there are polyps. It was unexpected and scary but it turned out it was necessary. They found two polyps and Stage 2 endometriosis, both of which MIGHT be impacting fertility. 

    It took about 2 weeks for fully recovery.

  • Hi! I did not see your initial post...sorry! I am glad your RE recommended the laparoscopy before starting the treatment, I wish my first RE would have done that and saved me time, money and **TW loosing 2 babies and 1 transfer that did not take**TW.  I have been on endo treatment for years before trying and my first RE assumed all was ok.  Then I changed to a smaller practice and my new RE said it would probably make a difference having everything cleaned - they found endo, polyps and a small fibroid-.  I am 18 weeks now :)

  • Hi! I can't help with personal experiences. In this article, they say IVF is the best option but the decision should be made on the best knowledge. If you trust your doc, you are in the best hands. Good luck
