New here- first round IVF w ICSI (warning- failed IUI mentioned)

Hello everyone- my name is Sarah and I’m new here. Ive never been pregnant and have no children. My husband and I have been trying to conceive for 18 months. We have “unexplained infertility” and male
Infertility factors (low sperm count). We did 2 unsuccessful IUIs. We will be doing IVF w ICSI this month. I am looking for any support, tips, and tricks that people are willing to give. Sending lots of love and support and baby vibes everyone’s way ❤️

  • Hi! We TTC for 5 years and are finally pregnant after doing ICSI! I have PCOS and my husband has low motility. My biggest piece of advice is to make sure you have a great support system. The shots aren’t bad (except for the PIO.. it kind of sucks. I highly suggest laying down when you get those shots. It helps your muscles relax more). Give yourself grace during the process, it’s mentally draining. Ask your doctor all of the questions. Also, don’t get discouraged. There were so many points during our cycle that things were not going like we or our doctor thought they would go but everything worked out the way it was supposed to!

    I'm in a similar boat for my first round of IVF with ICSI this month. We had 4 failed IUIs.

    I'm on birth control now. Starting stims on April 22nd with a retrieval early May. I'm anxious about it all but also ready to get started. Would love to support you however I can as we go through this together! 
  • Hi! I am also at a similar stage in the IVF process as you all are. I have PCOS and my husband has low sperm counts and low motility with a genetic factor.

    We started a stim a month ago and had to cancel half way through because of two dominant follicles. I was not mentally prepared for this as a possibility so I took it really hard. I’m sure all of the hormones in my system did not help. Stay optimistic and hopeful but ask questions so you understand what the docs are looking for and observing. I am now on birth control waiting to try again.

    I agree with laying down for the shots. I found that was easiest and luckily I have my husband to help administer them. I have done Gonal F and micro HCG and those really didn’t hurt much. It was more the anxiety in my head. I did have a bit of discomfort from bloating, so get some comfy pants or dresses. :)

    In my experience friends and family can have a hard time understanding what I am going through, so I recommend having a therapist in your back pocket if you are open to that. It’s just nice to know you have someone who can help you get through all of the ups and downs. I’ve found it helpful to have a judgement free space to say what I’m thinking and learn how to process it so that I can stay motivated to try again. This community seems like a great place to start though!

    I stopped the BCP on Friday and have my scan tomorrow morning and will be starting meds as long as I get the all clear. I believe I’ll be starting w follistem than adding menopur a few days late.
    My trigger shot is an IM hcg- definitely something I cannot give myself. My husband is nervous but said he can do it (fingers crossed!!) I’ll definitely take the advice of lying down for that butt shot! I heard icing in advance can help too.
    I was not given progesterone shots but vaginal suppositories instead- I’m relieved to be done w the injections but I heard the suppository can get quite messy
    Please keep me posted of your journeys ahead! I agree- I have very supportive friends and family but it’s hard for them to really know what this is like. Sending good vibes to everyone ❤️
  • @microlady - so sorry to hear about your cancelled cycle. I didn't know that was possible either! Thinking of you as you try again! 

    One thing I've been doing is saving some fun activities until I started this cycle so that I had a few things to look forward to. I have a Lego project, some other graphic design projects that I haven't had time to get to, etc. My sister also just bought me this coloring book. It's so funny! Highly recommend. 

  • @knottie I definitely recommend getting your husband involved with the injections. If you have you do them yourself, you can absolutely do it! But, I feel this is a great way for your husband to be a part of the process. With all the daily ultrasounds, blood work, and injections it’s a lot for you to go through. This helped me feel like he was going through it with me and we were doing this together. If I was feeling crummy that day or nervous about the injection, he would give me words of encouragement while administering the shot. Your husband can do it and will get really comfortable with it. Mine was nervous the first day and I gave him some words of encouragement to get through that and he was a pro after! We also prep the meds together so we have two eyes to confirm we did it right.

    For the sub-dermal injections I would lay down and pinch the skin while he swabbed with alcohol and gave the injection. I put on a tv show or podcast to distract me a little. I plan to try icing the area next time because I read that helps numb it, but it’s not necessary because these ones are a small little pinch if you feel them at all. I did notice as I got more bloated it was harder to pinch the skin and my skin was bouncier so took my husband just a little more pressure to insert the needles, but they really just slide into the skin. You don’t need a dart throwing action for these ones! We injected everything slowly and I could not feel it.

    I have not done intramuscular yet. I have read to keep that area warm so the muscle is relaxed, insert the needle quickly and inject slowly, taking breaks while injecting if you need to let the medicine disperse and make room for more. They also say to massage the area after for a few minutes and do some light activity to keep your glutes moving to prevent knots and help with soreness.

  • Loving all the tips and tricks being shared. Very sadly, our cycle this month was cancelled. I am devastated and trying not to totally lose it. My husband tested positive for Covid (we are both vaxxed) I’m sure I will have it within a few days. So heartbreaking. 2 years without getting it and during the most important month of our lives AND quarantining except for work but somehow he got it. I’m trying to stay positive for next month but this is one of the lowest I’ve felt during this whole journey 😢
  • @knottie190fcad4af29d763 I’m so sorry you had to cancel your cycle. :( I just went through a canceled stim after 6 days of injections and I was devastated. Mine was canceled due to other reasons but I am always worried about getting COVID in the middle of this. I wish I had some advice to help take your pain away.  I don’t know if you had started your injections yet, but I just let myself cry when I needed to for a day or two. I don’t know if it was the estrogen levels dropping or just time but I started to feel better after a couple of days. Just leaned on my husband for support because I didn’t know about this group yet.  I’m still having a hard time but trying to stay optimistic for the next cycle.

    I hope you’re husband has a very mild case and that you are able to stay healthy.  COVID on its own is stressful enough. Take care of yourselves and I hope you can start your cycle again soon.
  • @microlady thank you so much. Sounds like we’re in a very similar boat this month.
    I actually was supposed to start my stim meds today so I never got that far (positive test yesterday).
    Is your plan to proceed w another cycle next month? That’s our plan- I’m worried about what will happen to my cycle after taking birth control for 9 days, I know I need to be patient but it’s getting harder and harder. I hope we both have successful results next month!! No more cancelled cycles!
  • @knottie190fcad4af29d763 As much as I don't wish anyone to have to go through this, it is nice to not feel alone.  My stim was canceled on 3/25/22, so I'm on progestin birth control for two weeks while we wait for my ovaries to calm down again!  I have a baseline ultrasound and blood work scheduled for next week on 4/20/22.  If everything looks good we will start stimming again on Saturday 4/23/22.  I am cautiously optimistic. :)

    I hope that not starting your meds and instead being on the birth control for a little longer actually helps you get back on track sooner.  I'm usually a very patient person, but infertility has definitely tested that and I really dread waiting and delaying now.

    Yes, I hope our next stims are successful and we can then look forward to transfer!  Are you planning to do fresh transfer or FET?  We are doing FET because we need to do some genetic testing first.  Keep posting updates!

  • @microlady I am sending all my good vibes your way for you to resume the plan this month! I’m sure With a tweaked regimen the result should be better! That’s so exciting you get to start up again in a few weeks. 
    I will have to restart the process next month but it is what it is. Patience is definitely hard to come by but it’s out of my hands. The plan is for a fresh transfer as of right now. My understanding is depending on how many fertilize the transfer will be 3 or 5 days after retrieval. Now I’m just hoping this month goes by quickly and my cycle isn’t totally out of whack for that week if birth control I took 🤞

  • @enaids, I don’t know why, but I feel SO much better than I did last time. Today is day 9 and we think we will trigger tomorrow but I have an appointment in the morning for ultrasound and lab work to make the final call.  We are currently watching about 15 follicles growing :) which is wonderful because we have a genetic component that will likely result in more aneuploid embryos.  We are anticipating a Tuesday retrieval. 🤞 

    How are you doing with your stims?  That’s pretty cool that we started on the same day :)

    @microlady yes! Almost all the same for me. We just did the trigger shots tonight. I've got a COVID test and bloodwork tomorrow morning and retrieval on Tuesday too!

    We have 13 follicles right now which I'm a bit disappointed about, but trying to focus on the positive. This is our first round so didn't exactly know what we could expect with how many so we'll see how many they retrieve!! 

    Sending all the positive vibes your way for your retrieval and the waiting afterwards to hear how they progress! Keep me posted! 
  • @enaids. How fun that are cycles have been so similar! I was hoping for more too and so was my doctor but I think over 10 is still great :) I’ll be wishing you the best of luck for your retrieval and sending positive thoughts for your growing embryos!  Keep sharing :)

    We're hopeful that a lot of these make it to Day 5 blasts!! How did yours go? How are you feeling? Are you planning to send yours out for genetic testing?
  • Hi @enaids that’s amazing news!  I’m so happy for you and will be sending good thoughts for your growing embryos!

    Unfortunately, we learned that I may have an egg quality issue (I’m only 32!) on top of everything else because we only had 2 mature eggs out of 12.  One of those fertilized yesterday and we will find out about the other one today to see if it will fertilize after another day.  3 more matured in the lab after 24 hours so they did ICSI on those yesterday. We will find out today if any additional fertilized. So we are watching 5 right now and hoping for the best.

    If any make it to the blast stage we will be sending them out for PGT testing.

    I'm feeling sad and bummed, but trying to stay hopeful for the five we have. We will see what the embryologist says today.
  • I know it’s very easy to feel discouraged, but don’t! Our baseline showed 32+, none of my eggs grew after 2 weeks on gonal f and had to up my dose and take for an additional week and a half. Only 5 ended up growing, only retrieved 4. They told us we would probably lose 3 out of 4, if not all 4. We ended up having 3! We transferred 2 and have a frozen embaby!

  • @enaids thanks for checking in. I’m feeling extremely fragile right now and having such a hard time handling any hurdles.

    We were only able to biopsy 1 embryo for PGT-SR.  We chose the PGT lab because our dr recommended it due to the unique testing they can do for cases like ours, but there has been miscommunication between this lab and the embryology lab because it’s a new relationship. It just makes me doubt whether our single biopsy is in good hands and makes me so nervous.

    How are your embryos doing? I hope you were able to biopsy several blasts!

    @clwilkes thank you for your post, it has really been helping to remind me that there’s still hope that our 1 embryo will be healthy and we can move forward with the FET.

    Oh I'm so sorry to hear that. That feels so devastating, and with miscommunications between the lab that's so hard to handle. I hope that they'll be able to transfer that embaby of yours!! You're strong and you'll get through this!

    We ended up with 4 sent for testing. At each step I keep saying anything greater than 0 is enough. I'm worried for the test results because we've had one chemical pregnancy and then nothing for a year and a half now. So not sure if that had been a genetic thing or not 🤷‍♀️

    What are you doing the next couple weeks to keep busy? I'm already feeling anxious to hear the results. 
  • That’s great you have 4! I hope your genetic results come back normal for most of those and that you are able to have a successful transfer.  I have never had a positive pregnancy test, but I can only imagine how frustrating and devastating a chemical pregnancy feels.  I’m sorry you have experienced that.



    Luckily work is picking up a little bit so I can focus (or try to) on that during the day.  I don’t really have a game plan right now for distracting myself but I’ve been trying to complete some projects around the house.  We may start a mock cycle for ERA while we wait for results, but not sure yet.

    @microlady and @enaids - thinking of you both and hoping your results come back normal and you are able to move forward with the process. The waiting sucks and it’s so hard not to feel anxious even when you try to distract yourself. We got our results back but are still waiting for FET (can’t do anything until my next cycle starts and of course this month it’s a few days late but I guess maybe that’s to be expected in this process?). I’m glad you both had at least 1 that was able to be biopsied and am hoping for positive results for you. Sending positive vibes and lots of love!

    We just got our results back and we have 3 genetically normal embryos out of 4. We're so excited. We may have to wait an extra cycle to do a transfer though because my clinic basically shuts down the first two weeks of July. Right now depending on when my next cycle starts we may be able to do a transfer around the last couple days if June, but if my cycle starts a day or two late then we may have to wait another cycle which is disappointing. I'm debating going back to acupuncture to see if there's anything they can do to shorten my cycle. I guess we'll see though. 🤷‍♀️

    @enaids that’s great news about your genetic testing! I hope your cycle starts on time so you don’t have to wait for the next cycle. Mine finally started but was late so that’s pushed our timeline back a little. Right now just going in for periodic lining checks so hopefully we’ll know more soon. Hang in there!

    @medp87 is that normal to start your cycle later then expected the full cycle after egg retrieval? My friend's is like 6 days later than it was before. I'm worried that's the norm after an egg retrieval.

    I just wanted to write in to give hope to all the mothers there. You all are great. Couple of pieces of advice. First, if things don’t go as per plan, don’t get disheartened. Say a prayer and move on to the next day. One day at a time. Don’t think too much in advance and worry yourself. Second. I know this is tough, but try and relax. It helps. Now my story. Tried naturally. Had a miscarriage. Tried again, we had to abort at 17 weeks due to trisomy 18. To this day, I’m devastated. We then tried for 18 months naturally and gave up. At age 34, we decided to go for fertility treatment. We were given all the stats in the world and along with that all the hopes. Two IUI’s failed. Blood tests showed low AMH levels. 0.8 I think it was. Only hope was IVF. 7 eggs were retrieved. 5 mature and tried to fertilize. All of them failed. We were then given options to use a donor egg or go for second IVF(1% chance). We decided we would go for the second IVF. Since we knew the odds were low, we did not stress much. Enjoyed life. And then a miracle happened. Got pregnant naturally and now have a healthy 22 month old. We went through a lot of pain and as I type I feel like crying…So relax, things will work out one way or the other…

    @enaids I’m not sure….I thought it was weird that the first one after retrieval was normal and then this one was so late (by about 11 days).  When mine was a week late they asked me to come in for US/bloodwork to see if I had ovulated and since I had they told me I needed to wait it out. I think if I hadn’t they may have prescribed something to push things along. Have US tmrw to check the linking and see when I need to come in again. Hoping your cycle gets back on track so you can get to the next step! 
  • A failed IUI can be devastating for you and your partner. You may feel lost, and profoundly depressed. But, you aren’t quite ready to give up your dream of growing your family. The decision does vary from patient to patient, it is common for patients to progress to the IVF treatment process after they have had failed IUI treatments. Intracytoplasmic Sperm Injection (ICSI) is an advanced reproductive procedure done in an embryology lab during an IVF treatment in which a single sperm is injected into a woman’s egg in order to create a fertilized embryo. IVF-ICSI is one of two fertilization methods that can be used during IVF treatment, the other being Conventional IVF Fertilization. Once the eggs have been retrieved during a procedure called an egg (oocyte) retrieval, it is time for the true IVF to begin. Eggs can be fertilized via ICSI or conventional insemination.
