Previous pre-term pregnancy and effects on current

I figured I’d make a post on this because I wasn’t sure where it belonged...

I had another appointment yesterday. “OB history” appointment. My DD who is now just shy of 5 years old was born at 36 weeks and 6 days. My labor and birth process was only a total of 8 hours however fairly traumatic. Anyways, she was still considered pre term even though it was only by ONE day.

my question is this... my doc told me yesterday they want me to start progesterone injections at 16 weeks for the rest of the pregnancy because of the above. To me that seems a little outrageous. I can see if I had her at like 30 weeks or something but was wondering if this has happened with anyone else or what your thoughts are? I would really prefer not to take these injections if I can help it.

  • @cait32 thank you! That’s my thoughts. The doctor I am using now is different than my previous one I had my DD with. I had her in NC and I’m in Ohio now. There’s multiple CNP in the practice I’m seeing now maybe I will ask a different one but yes she was aware it was only early by one day and no real reason. My labor was just random. I did have a lot of edema towards the end and they were going to write me off work but no high blood pressure, no cervix issues or anything so I felt it was kind of crazy to think I needed those injections.

  • Slightly related, but my practice is having me start in baby aspirin to help prevent preeclampsia again this time around (which I developed 72 hours postpartum last time) even though my blood pressure was 115/56 at my appointment on Monday. It drives me crazy how overly cautious they can be, which I’m sure is the case for you. I’d almost rather get a shot of magnesium if labor does start early or opt for steroid injections towards the end to help with lung development in case this babe comes “pre-term” again.

  • @chindimples04 I’m also starting baby aspirin to prevent preeclampsia. I had gestational hypertension late in my last pregnancy. From what the doctor said, starting baby aspirin early can help issues with the placenta that cause high BP, it doesn’t actually lower the BP. My BP is currently super low but I’d do anything to avoid the issues I had last time. Not as extreme as starting progesterone shots though!

    @k_rn21 My daughter was born at 37.5 weeks so technically full term, but she was only 5lb 2oz so they called her a preemie anyway. The doctors haven’t had any concerns about that as far as what to do this time around - the only thing I’m doing is trying to keep my BP low as mentioned above.
  • @k_rn21 I think it makes sense you would want to ask for a second opinion! I thought I’ve heard of the progesterone shots if you have cervix issues that cause preterm, I had my cervix checked during my last pregnancy at MFM via ultrasound bc I was higher risk for those issues bc of previously having a septum wall. My cervix was fine though so idk if they would’ve suggested the progesterone if it wasn’t. Either way you should always feel you can get a second opinion on anything! Maybe even ask if you could consult with an MFM doctor since they know more about “high risk” things than your OB would? 

    I had cholestasis with my son so had to be induced at 37 weeks on the day. Just saw MFM today and already since I had him in September 2020 things have changed in what is recommended. If I get cholestasis again but my levels are on the low side like last time they might let me go to 39 weeks. Crazy that in a few months time recommendations can change 🤷🏻‍♀️
  • @grangerhp37 mine was 28 weeks exactly when I was diagnosed with hypertension. That part wasn’t the worst, and my pre-e wasn’t all that terrible either, but I would like to not be in the hospital for a week this time. Definitely not as extreme as PDG shots but the extra precaution idea stands.
