Hello and welcome to our first AMA/High Risk Check-In! I figured this was ok to combine since most OBs consider AMA as high risk. Let us all be a support for each other because we all know pregnancy is hard even without the extra complications! I will make a note after this week and tag you when I post on Tuesdays!

Re: *AMA/High Risk Pregnancy Check-In* Week of 10/6
What brings you to this thread? I'm AMA (37 years old), have had two previous losses, and have low progesterone. My BP was a bit high at my first appointment which sent everyone into a tizzy as well, so this should be an exciting few months, lol.
Do you already have a plan? I have a new OB this go around and he seems to be extra cautions, so AMA and previous losses seem to be prompting some additional ultrasounds. He called on Saturday to tell me my progesterone is low, so he put me on oral Prometrium 2 times a day. I assume if my BP continues to be on the high end, he'll put me on BP medicine.

*POSSIBLE TRIGGER* - MENTION MC
What brings you to this thread? I will be AMA by the time I deliver and have had 3 MC and one chemical pregnancy so far with no living children yet. Through RPL testing, I've been diagnosed with antiphospholipid syndrome and hashimoto's thyroiditis, both of which increase chance for complications. My second pregnancy ended and through my D&C the babe was diagnosed with Trisomy-18. In my 3rd MC, my thyroid TSH went crazy - from 0.01 up to 16 which is very high. I also suspect some progesterone issues due to inconsistent luteal phases and borderline results when I had a uterine biopsy performed to look at lining thickness.
Do you already have a plan? As much as I can at this point! Currently I have an OB, a hematologist, and an endocrinologist on my "medical management" team. I'm regularly testing TSH and free T4 and managing with synthroid medication. For the antiphospholipid syndrome, I'm on daily lovenox shots to the belly and a low dose aspirin regimen. I'm also on Crinone 8% progesterone cream - that does some weird stuff! Not doing too much from an OB perspective right now except trying to survive through the 1st trimester. As I progress, we'll take additional monitoring and testing as needed. I'm eligible to have the genetic testing bloodwork due to my age and RPL history, so will get that after 10 weeks! I had two medical ultrasounds around 7 and 9 weeks this pregnancy, but my next one isn't until 20w so I'll def be scheduling some private scans! So far everything's looking good - trying hard to stay positive ♡

I have type 2 diabetes. Last pregnancy I had GD and delivered early because of low amniotic fluid. My pcp and obgyn have both said I’ll be referred to high risk.

@mkc3888 How early did you deliver? Were you able to manage your GD with diet or did you have to have insulin?

What brings you to this thread? I’m 36, birthday is May 1, so I might be 37 when baby is born. Though it’ll probably be early because it’s a scheduled c-section... my 4th. That’s my other high risk.
I had pre-e with my first full term
pregnancy, but I also ate like crap because I was young and ignorant. Sometimes I get high blood pressure towards the end of pregnancy, I had it by 5 months with my first. Exercise and good (overall) diet helps keep it reasonable.
Do you already have a plan? Uh no. I am at the same OB office as I was with my last two, but my Ob retired. I already miss her! So my appointment is on Monday with another experienced ob, and we will see. I have a feeling that the scheduled c-section will be earlier than a week before due date because I went into labor the day before my c-section last time. My labors are real, but unproductive and tend to lower my baby’s heart rate.

What brings you to this thread? My water broke at 20 weeks during my last pregnancy which was obviously too early for our little baby... We don't know for sure why that happened: infection seems unlikely because everything came back clear, but I was never checked to see if my cervix was open when I got to the hospital so we can't exclude an incompetent cervix.
Do you already have a plan? I actually just got one! I had my first high risk OB appointment today. We really liked her and she recommended we go ahead with an elective cerclage at 13 weeks, so looks like that will be the plan to give us our best chance at a healthy full-term baby. Deep down, that's what I wanted us to do so I'm happy she preferred this approach to the wait and see (cervical lengths US and progesterone) as now I feel like we're doing the best we can. Now to accept the fact that I'll never have a "normal" pregnancy, but the idea of a good outcome is soooooo worth it. Now I'm a bit nervous about the procedure, but I think I'll feel more confident afterwards. I actually already feel more confident.
What brings you to this thread? I'll be 39 this month, had GD last time. I had one mc before my daughter, which I don't think is enough to count as high risk, but boy does it trip me up.
Do you already have a plan? Not really. I'm mentally preparing for all the GD baggage again and started exercising, but that's mostly because my clothes don't fit and I'm trying to squeeze another two weeks out of them, but really I can't do anything until I get the US saying if it's even a real pregnancy or not.
What brings you to this thread? I’m 37 & had a blood clot when I was 3 years old so I’m considered “high risk” because of that. During my last pregnancy I was on lovenox from 12 weeks until 6 weeks PP. I also developed pre-e and was induced at 37 weeks.
Do you already have a plan? Started lovenox at 8 weeks and will start baby aspirin at 12 weeks. I am on synthroid all of the time and they’ve upped my dose twice already. My doc is very proactive and understands my history. I feel confident with our plan.
@yellingbanana - Yes it has been! But at least with getting so many answers, there was always a path with a "next step" that I felt would get me closer to a healthy pregnancy, so it's definitely helped me stay focused! I also see that we're due date twinsies for now 😁 I hope everything will go smoothly with getting things scheduled out.
How far along are you? 8 weeks 1 day
What brings you to this thread? I’m 36. I have a history of miscarriages but not enough to be considered high risk from that.
Do you already have a plan? I have been working on a plan. Right now, my plan is to have a repeat c-section about a week before my due date and have my tubes tied during. Both my boys were born having seizures and I’m working on getting a game plan for that just in case baby #3 has the same issue.
@mkc3888 Hopefully you'll get all things GD worked out early on this time!
@mimser I'm so happy you already have a plan. I can absolutely imagine you'd be nervous, but hopefully, in the end, it will calm your nerves!
@queen_frostine When is your US scheduled for? Will they do an early glucose test due to previous GD?
@samd6 I'm so glad you have a proactive doctor! It definitely helps with the confidence!
@ruby0902 Have you gotten your bloodwork results yet?
@dlynn484 I asked my OB about permanent birth control because our plan is for this to be our last kiddo. He suggested waiting and doing a laparoscopic tube removal surgery a few months later. I asked the reasoning and he said doing it during a c-section, you have more of a risk of bleeding because the increased blood supply in the area. Plus, a tube removal has proven to lower the risk of ovarian cancer. Just something to think and ask about! I still need to research it myself but found it very interesting.

What brings you to this thread?AMA I turned 40 a week after we conceived. This was not planned (had first kiddo 6 years ago and thought we were done) and I probably wasn’t taking the best care of myself at the time. Also my husband is super worried about chromosome stuff. (He’s 49).
Do you already have a plan? Hoping to go the birth center route as that’s what I did with the first one. Starting out at the hospital clinic here though- just had first ultrasound today (measuring right on and string heartbeat- yeah). Have a genetic counselor consult later today... just very overwhelmed
as none of these options/ questions/ doubts Came up the first time. Oh- and I’ve been spotting constantly the past 4 weeks so the good ultrasound today was a relief!

Ultrasound is the 19th and I guess I'll find out then if they want to do an early glucose test.
