Re: August 2020 Mental Health Check-In

  • Question for you lovely ladies - my therapist and I did get into a discussion about medication as the fights DH and I end up having seem to occur right before my FW so she thinks there could be something hormonal going on (clinical depression).

Her suggestion was to discuss it with my OB or with my PCP (which I don't really have, but there is one doc I've gone to a few times in the past for things who you could say is my unofficial PCP) to get prescribed. So, who would be better to go to? I get talking to the OB since it appears related to my cycles, but like... can they even prescribe anti-depressants? Should I talk with a PCP?

I'm also not sure how comfortable I feel about talking with my OB (a man) about it. I do need to schedule a pap though.... My PCP, she's a super nice lady and I really like her so I feel I would rather talk to her and also think she would be more accessible than my OB (who is at different locations on different days).

I did think it was a little weird that my therapist didn't suggest talking to a psychiatrist first though. Is that normal??

  • @inthewoods23 I am a PCP and can confidently say they could definitely prescribe something. It is pretty common for us to do, and we often do it the same visit someone is coming to establish care, so it doesn’t even really matter if you have seen the person before. It sounds like you may have PMDD (premenstrual dysphoric disorder) which is a variant of depression that gets worse before periods. An OB can theoretically also treat this, but sometimes they will send patients to primary care for things like this. In your case, I would definitely talk to your PCP. Regarding a psychiatrist, they are rarely the first step. We usually don’t send someone to psych unless they have a more serious diagnosis (schizophrenia, Bipolar, suicidal, etc) or we have tried multiple meds that don’t work.   TLDR: either is fine, PCP could definitely do it.

For someone trying to conceive, they will almost certainly recommend Zoloft/sertraline as it is best studied in pregnancy and what you would be switched to when you get pregnant. Frequently if someone is on a different med, the OB will send them to me to switch to Zoloft if they get pregnant.

    AFM:

Mental Health Diagnosis (if you have been) or What brings you to this thread today?: the diagnosis in my chart is “situational depression” but who knows

Status (WTO/TWW/Benched/TTA): Benched in what would be TWW if I was actually waiting for something

How are you feeling? Super nervous about meeting with MFM tomorrow to get results from genetic testing that will determine whether we can continue trying naturally.

Where are you in your mental health journey? (in treatment, looking for support, on meds, in counseling, having a rough day, etc.) All of the above? I am having a rough day, I am on treatment (Zoloft of course), I’m in counseling, and looking for support. I have been semi-MIA while benched, but keep coming back to the TTC board for the support of you lovely ladies. It is nice to know you are not the only one struggling.
  • @fitzfizz thanks for your input! I will work on getting an appointment with my PCP and hopefully get prescribed. I hope your meeting with the MFM goes well tomorrow!

May as well add the things. I haven't needed this thread lately, but I'd like to keep the conversation going.

Mental Health Diagnosis (if you have been) or What brings you to this thread today?: Anxiety and depression

    Status (WTO/TWW/Benched/TTA): TWW

How are you feeling? I really really struggled discussing my feelings today with my therapist but I got through it. It's so easy to type it out to internet strangers, but it's a completely different animal to confess to someone's face that after a fight with DH on the way home from my sister's I thought over and over about stepping into the kitchen, grabbing a knife from the knife block, and stabbing my leg over and over. And I was the one driving! Obviously I didn't do that when I got home... But those are the kinds of thoughts that develop in my head. Never acted upon, I know better. But it's pretty clear to me I need more help than sessions with a therapist can provide.

Where are you in your mental health journey? (in treatment, looking for support, on meds, in counseling, having a rough day, etc.) In counseling, and now working on the process of getting prescribed some meds 🙂

Which is good because my mind when to sad places this morning and I started crying in the shower. Walked to the bedroom sniffling and laid in DH's arms to cry it out for a few more minutes, then I was feeling a lot better. Now I can't wait for Monday!
