July 2019 IUI/TI

I haven’t seen a board yet for July IUI/TI so I figured I would start one up. 

Here we go again! After Menopur left me with a giant cyst last month I was able to start back up again. I responded much more quickly this time, but not without something weird happening (of course)! So I started a nice little  Menopur and Gonal-F cocktail last Thursday with a baseline E2 of 35. Four days later estrogen up to 161 and I had a 13 and a 15 on the left and an 8 and an 13 on the right. Then  Wednesday I came back and the left had a 17 and 12.5 and the right had a 10 and 12.5! Then today a 19 and a 13.5 on the right and an 11 and 13.5 on the left. So they shrank and are now slow to grow. Has anyone experienced this before? They want me to trigger tonight so we don’t lose the 19 and she keeps saying anything over 14 can inculcate a mature Oocyte (which by tonight should be close to that... but I am so nervous I’ll only get one egg. After 4 large follicles and a failed cycle last month I am SO NERVOUS. Anyone have any advice or stories to offer?? Oh and my E2 has gone up nicely, 693 today

    I’m in a testing/monitoring month. I had my saline Sono on friday and it went so well. The last one I had 3 years ago was awful - this time I felt almost nothing. 

    I went in today and I have one follicle on my right that’s 17mm (with my 12mm endomitrioma) and my E2 is at 645, lining is 9 which is great. I wish the follicle was a bit bigger but it may still have a day to grow before I ovulate. Bloodwork showed I’m surging and the clinic said it’s worth trying out TI this cycle so...the deed has been done lol. Beta in two weeks! 

    @Steele743 - having one large follicle like that is great. That’s what you want. Your E2 number shows there’s one dominate follicle too. One strong follicle is better than a few weaker ones - 19mm at trigger is great!!! Worked for me last time :smile: Good luck!!

  • Hey @Steele743 I am not too familiar with shrinking follies but I am pretty sure that having one @19mm is great! I know we all hope to have more than 1 but really that's all you need. In fact, I've seen stories of women who have had failed with 3 follies then were successful with 1 so stay positive. Good luck and keep us updated. 
    Hi @JWatt5 I hope that your ti works out for you . It would be amazing to work on a testing month. You said it worked for you before? Was that from IUI? Also what protocol were you successful with?
       Afm, went in for day 3 scan and looks like we are good to start folistim tonight. I was very fortunate the last cycle, my RE provided the folistim bc they had some donated but this time, its all out of pocket. Just trying to stay positive on this infertility journey but its so hard. I feel upset but then even worse when my husband tries to hide his disappointment when I tell him month after month that it didnt work.

  • @JWatt5 I know, lol. One is so good!! I’ve just always had at least three so I was a bit pessimistic I suppose. But I’m 6 dpo now and I feel really good! The last time I had about 6 mature follies and I’m almost positive I ovulated early so we missed the timing. I was left with a ton of hormones and a giant cyst and I was in so much pain!! So this is all good!! 

    @bobella22 I am so excited that you’re beginning your follistim!! Hopefully you respond well and this is your month!! I know it is so hard on our husbands as well. I’ve seen mine hold back tears many times when I break down after another negative. They really love us and we are so lucky! Infertility takes such a toll in every aspect of life! 

    So I have a random question- since last time was such a mess for me, I requested a 7DPT serum progesterone mostly for peace of mind. It was 43.7. They told me that Endometrin is only absorbed through uterine tissue and isn’t reflected in blood serum, but it seems high. Any thoughts or ideas on this? Or does anyone know how to interpret these results as far as likelihood of pregnancy? Or is it MERELY a sign of ovulation. I wish I wasn’t so obsessive lol

    @bobella22 - it was IUI with injectables - with Stage 4 endo, we didn’t think that would happen. My first cycle with injections didn’t even make it to the IUI. I ended up with a cyst that kept the follicles from growing at all - it disappeared on cycle day 11 - they though it had been the lead follicle for a while. The second cycle went better. Started with 50iu Puregon(fsh) and then cycle day 9 to 13 they increased it each day to get the things moving. I think the last injection was 100iu. Slow and steady won the race - triggered with a 19mm follicle and IUI the next day, IT the following day. I did ask for progesterone suppositories just to be safe. 
    We’re hoping this time around with pregnancy and Mirena since 6wpp I’ve kept the endo calm to have another success story. There’s a few girls from PAIF from 2016/2017 that had a surprised natural after with various IF issues.

    Hey guys! I’ll join you over here.

    I’ve been on 5mg of letrozole cd2-7 and I’m actually nervous about how much they’ve stimmed me this time. Our issues are sperm related, so I’m worried I’ll have a lot of eggs this time. This will be our first IUI but our 4th timed med cycle (one success but loss in there). I’ll also be tiggering for the first time. 

    I go in monday at CD12 and I’m hoping timing works out for Friday IUI but I’ll take a successful IUI over a convenient IUI. 

    @JWatt5 that’s great you got a TI cycle out of the monitoring month - hopefully you don’t need to go back :) 
    @bobella22 MH is certainly the more emotional of us so I understand how hard it is to see him suffer. Hoping you respond well this cycle!
    @Steele743 I have been told progesterone can vary wildly and that as long as it is above 10, it’s okay and you ovulated.

  • How is everyone doing so far this cycle???

    AFM I have a 25mm follie (only one this time) and 8.8mm lining on CD12 and I am waiting for trigger instructions based off of blood work from this morning. IUI is 8am Wednesday. They did a post-coitus CM check and rated it "poor", so they're very happy I'm doing an IUI because my CM is NOT responsive to sperm at the moment.

    @chichiphin - that’s a nice lining measurement! 

    Im getting my hopes up too much I think that this could actually work. I know my follicle wasn’t a great size when I was surging. I cramped up yesterday which is super weird but I also know it’s too early for implantation cramping. Didn’t miss this TWW symptom spotting lol.

  • @chichiphin I can’t wait to hear how your IUI goes tomorrow!! I’m keeping my fingers and toes all crossed! @JWatt5 what DPO are you??? I’m ok 10dpo and I haven’t tested for once!!!! Might cave tomorrow. Ugh!!! But I’ve been super crampy today so I’m thinking I’m out
  • I am just at the beginning of my cycle but I would love to join you ladies. This will be my first IUI cycle (if the timing works out). We will be out of town July 19th but the NP felt we would still have enough time for IUI before we leave. Fingers crossed that is the case. This will be my first time using Follistim. Does that normally make you ovulate sooner? My cycle has been really crazy the past 3 months and I have been ovulating later and later. The plan is Femara CD4-8, follistim CD8-9, Ultrasound CD10. I honestly don't feel like I will be ready on CD 10 so I am considering calling and at least pushing it back one day. I normally ovulate around CD 15-17. May it was CD 21. Last month it was like CD 35. I had been doing low carb for a few months so I feel like that was the cause. I am not longer doing that in hopes that things will be more normal.

    I also had my baseline labs/ultrasound today. All of this is new to me so I will post my results and perhaps there is more insight.

    @mint2b5 welcome! I’d keep the CD10 appt if you can - you can tell a lot off of follicle size from one ultrasound and where it might be... and you might be ready to trigger by then! I’ve had follies over 18mm at that point on higher letrozole doses

    @Steele743 - I’m 6DPO. I’m planning to test 10DPO before a wine tour though. May as well get it over with and enjoy my tour. Then I’ll test again the Monday at 13DPO. Don’t count yourself out yet. Cramping can be a good thing. 

    @chichiphin - hoping for a unicorn so bad. Not usually that lucky though. I have to go next month with no monitoring/natural because my follow isn’t until early August. Argh

  • I’m back in the game! This is my 4th IUI attempt. We had to skip last month due to a large cyst that was discovered on my initial US. I start letrozole today (CD 3) and will be doing gonal-f 150 units on CD 4,6, and 8. He ended up adding more at my second US last time so we’ll see then too.

    @mint2b5 good luck with your first IUI! I would recommend keeping the appt on CD10 if you can. I tend to ovulate later but with the medications I seem to be ready sooner. Will you be doing a trigger injection? 
  • You guys. I took a test and it’s POSITIVE. Then I took three more to be sure. I’m in shock!! 

    @mint2b5 welcome!!!! Will they start doing ultrasounds on you regularly at cd10? Or just the one? I would trust your doc!! 

    @stillflying I totally know how hard it is to be benched because of a cyst. That waiting game is the worst! But it’s so important to let your body heal properly. I’m glad you’re back at it!! 

    @JWatt5 well a wine tour sounds amazing!! Lol!!! Sure you don’t want to wait until after to test? Lol! Can you cancel the tour or reschedule if you test before and it’s positive? 

    I really have my fingers crossed for everyone!!!! 
  • They will do a trigger shot. My RE office always brings you in at CD10 for ultrasound and then from there they schedule the repeat one based off what they see. The only cycle I was monitored I went at CD10, CD16 and triggered on CD17. I guess I won't put off the CD10 appt. I just feel like my body is slower to respond and I am being so vague with work. LOL! I am running out of excuses on why I am running late in the morning.


    @stillflying we are around the same point in our cycle!

    @Steele743 - congrats that’s great!? Do you have a beta scheduled? I’m not expecting a positive at all but if I did get one, I actually went on this tour with my friends when I was previously pregnant and had a blast!! They have non alcoholic wine at every stop. My friends all know we’re trying so either way it’ll be fun!!

    @mint2b5 - keep that CD 10 appt. with follistim they can adjust your dosage and that may really help. You probably won’t be ready but with injectables I found the more appts the better so medication adjustments can be made. My dose was increased every appt on the my cycle with it.

    I tested early with a FRER. Stark white. I know it’s early(9DPO) but also know the size of the follicle wasn’t great - bring on the wine tour lol.

  • @JWatt5 I had a beta yesterday at 13dpo and it was 193!!! I go back in for another on Tuesday 🤞🏻🤞🏻🤞🏻 I am jealous of your wine tour. lol. Even non alcoholic wine with friends is fun!!! Also, mine was super light at 11dpo and even at 13dpo with a super high beta it wasn’t like darker than the control or anything! You’re still in!!

  • Ahhhhh man @JWatt5!!! So when I took the frer on 11dpo it was a total squinted of a line! Like hardly more than an indent! Two days later beta almost 200. Try one more time! Lol. Is this consult for IVF!? 

    @chichiphin isn’t it the worst?!?! My progesterone is measuring super duper high, not sure if it means anything but I do know it makes me feel like poo!! I can’t wait for you to test in a week!! 
  • How’s everyone doing?? 

    I had had my follow up ultrasound today, CD 9. I had multiple follicles with the largest being around 17. My lining is also still too thin. They added more gonal f today and I’m doing a repeat ultrasound tomorrow. She said it looks good for likely IUI on Thursday or Friday!
    Also, random but kind of interesting, is that I knew that the gonal f pens are overfilled but no idea how to use the excess. Well now I know! It was too late for the pharmacy today to get extra meds so my doc explained how to use a different syringe to extract the extra medication. Was I alone in not knowing this??  So glad I kept the previous pens even though they were “empty.” Reminder to all! Don’t throw out the excess - save yourself the money!

    @Steele743 - I’m going to just wait for AF now. If it’s not here by Friday then I’ll test again but not holding my breath lol. How was your beta today?  My consult is to just start IUI again. This past month was just routine testing. IUI worked last time so hoping it does this time for us too. 

    @stillflying - yes with the excess lol. I knew you could get it out but I never did try to! that’s a great size for CD9! Hopefully that lining can catch up/they can get your E2 higher to do that!!

  • So I have questions because I’m a bit confused. As I mentioned earlier I had an ultrasound yesterday and was told my lining was thin and it would probably be Thursday or Friday before the IUI. I went back today for another check CD10 and the lining has barely changed at all (around 5), largest follicle is 20 (5 follicles over 10mm), and they had me trigger today for IUI tomorrow. So two questions: anyone had an IUI with a lining around 5? And every previous IUI I've had it occurred 36 hours after the trigger, not 24. Does anyone else do theirs after 24 hours? Unfortunately I couldn’t get answers to all of my questions since the doc is out of the office and they were communicating this with him by phone, but I’m nervous and just looking to see if anyone has had any similar experiences.

    I’ve never understood why some have the IUI 36 hours later. You want the sperm there and ready for when you ovulate. My clinic has you trigger at 4pm and IUI at noon the next day (and clearly it works, I have a two year old doing it that way). BD the following day too if you want. 
    You’re lining isn’t perfect but that trigger shot can help improve it still - with a follicle at 20mm it’s likely best to trigger now to give that follicle a good chance - you don’t want them too big either. 20mm is a perfect size for trigger. Usually you want the lining at 7mm or more but again...there’s like 48 hours until you actually ovulate so there’s still time to have it thicken up. Believe my successful cycle mine was 6mm at trigger. I asked them to check the day of and it was 8mm. Make sure you ask your questions with the clinic. They owe you those answers! I’ve been on these pages 3 years now...I really think your RE is giving you the best shot possible this cycle. Good luck!

    @stillflying - it’s hard not to obsess over every detail. While I thought I leant last time, I still do lol. Reality is, even the most perfect seeming month can be a fail and the one that feels hopeless can be it. So many different approached and protocols. We all just want it to work so bad. You’ll get there - this will all be worth it.

  • @JWatt5 hey!!! My beta went really well! So at 13dpo it was actually 196 and then at 17dpo it was 1480. Ultrasound in 2 weeks from today! I really hope you get a pleasant surprise on Friday!!! On the bright side, real wine on the wine tour! Please keep me posted!!!
  • @stillflying your follicle is a good size - hopefully the trigger gets you to at least 7! I did 36 hrs post trigger myself and my follicle was releasing when I showed up for the IUI. Will you be scanned prior to IUI?

    AFM - I stupidly tested this afternoon to see where the trigger was. I’m 7dpIUI and 9dpt and it’s still pretty dark. Ugh. I guess this means I’ll be in line hell for the next few days...

    AF is officially here. We don’t have our consult until August so this month we’d just be a completely natural/no monitoring month. Not sure what we want to do yet.

    Well my 5000 trigger is still here 12 days post trigger... I’m going crazy wondering when I can assume it’s no longer trigger. 

    Edit - it’s darker than the others. It’s back to the line I had at 5DPO....

    @chichiphin - ummm that’s a long trigger. Too long my lady. I don’t want to get your hopes up just in case but...don’t think that’s your trigger anymore :) mine was gone 9/10dpt last time. Fingers crossed for you.

  • I know I'm late coming in... my trigger unfortunately stayed in my system for 13 days last cycle 😟  but for your line to be that dark is very positive! 🤞🏼🤞🏼 baby dust!
    I triggered Friday with 2 follicles 1 at 19 the other at 21 I'm taking clomid 100mg cd3-7 and 75iu of follistim CD 10-13 triggered on CD 14 I usually O around 36-40 hrs after trigger.
