New to the Bump community - first IUI this week

So I'm 38 and DH is 36, we have been married for almost 10 years. We weren't actively trying to get pregnant the first few years but we also weren't trying to prevent it. I have irregular cycles so I figured it would take time. We actively started trying 4 years ago, we did opk and timed intercourse with clomid for a while. We finally decided that it was time to see a RE, and called for a consult. The consult finally came around in November. I was diagnosed with PCOS, which my GYN never diagnosed. The recommended that we start with IUI. We were excited to finally have a diagnoses and hopefully a good plan of action. 

I am trying not to get my hopes up though because I know that IUI doesn't have a high success rate, especially the first cycle. Plus I'm worried because I only had 2 follicles with the largest being only 15.15mm at my last U/S on 01/18. They had me do the trigger shot on 01/18 and we did the IUI on 01/19 which was cycle day 17. I go back on 01/25 for my first blood work appointment and then on 02/04 for my Beta.

I feel like the follicles weren't big enough and they did the procedure too soon after the trigger shot. I'm just scared and confused and honestly since it's my first time I don't know if I should be worried about those things or not.

  • Hi @afox1029. If I were you, I would express the concerns you have related to the follicle size and the timing of the IUI at your next appointment just so that you and your RE can be on the same page. To my knowledge, a follicle should be at least 20mm before the trigger shot is given. Plus, I don't know what RE would do an IUI the day after the trigger. Did they go over the procedure with you prior to you deciding you would move forward with an IUI? They should have outlined all the details with you.

    I'm guessing the procedure was done less than 24 (maybe even 18??) hours after you took the trigger shot. Is that correct? Everywhere I read says to take trigger shot at night when instructed, then IUI 36 hours later in the AM. However, I do know that some REs will have you do the procedure twice on consecutive days so maybe they had originally intended on you coming in a 2nd time and then they forgot to have that set up.

    Don't count yourself out yet though. Even if O was after the IUI procedure, the sperm would have been waiting for the egg at the time of O. GL!

    My RE does the IUI the day after the trigger as well - then suggests intercourse the next day if we want. Worked for us :smile:
    Edit to add - my follicle was 17mm when I triggered.

    @JWatt5 thank you for sharing your experience! It definitely makes me feel better about my experience. My RE also suggested intercourse the next day so fingers crossed that it works for me :)
  • @inthewoods23 I definitely appreciate your knowledge, it certainly helped to ease the worry a little. I was just freaking out a little bit. I am going into see my RE on Friday so I'm going to bring up the concerns at that time.
  • @afox1029 I'm right there with ya considering the money spent to just try for a baby! Timing is such a tricky thing which is the one worry I have with doing these IUIs!

    What the ladies have said already re-IUI --- the sperm will stick around even if O is a little bit after the procedure! Our doctor recommends intercourse the days following IUI if possible jsut to cover all of your bases :smile:

    We were several years TTC on/off (sometimes deliberately, sometimes not trying/not preventing) before going to the RE. You are in the right place, wherever your journey takes you - IUI success or on to other options. Sometimes I would write out a list of my questions/thoughts to take with me to an appt so I'd remember to ask the questions. I would always make sure I had DH or my mom with me as well to make sure everything was clear/questions asked/answered.

    The cost sometimes frustrated me -- why can't I make a baby for free like everyone else?! But ultimately, you don't care about the $$ in the end. 
    ~~ Our Story in Spoiler! TW loss/child~~
    Fall 2012 -- started TTC
    Summer 2015 - no BFP yet, labs normal, referred to RE
    Fall 2015 - Summer 2016 - Further testing all normal. 3 IUI's -- BFN. Recommended move to IVF. Planned cycle for fall 2016.
    September 2016 - Surprise natural BFP. MMC @ 8 weeks. RE expressed confidence that we just needed the 'right' embryo.
    Fall 2016 - Spring 2017 -- Break from TTC
    June 2017 - Started IVF; egg retrieval for freeze all cycle. 9 mature eggs retrieved, 5 fertilized. 2 4BB embies on ice.
    August 2017 - FET transfer both embies. BFP.  Twin pregnancy confirmed by ultrasound. EDD 4/28/18
    September 2017 - Twin B stopped developing; Twin A doing perfectly! Graduated from RE @ 10 weeks
    March 2018 - Baby Girl born via C/S due to pre-eclampsia -- strong and healthy!

    TTC #2
    January/Feb 2021 - Freeze-all IVF cycle 
    March 2021 - FET of 1 PGS normal female embryo. BFP! Beta #1 156,  #2 472, #3 1241, #4 5268 EDD 12/5/21 - Christmas baby!

  • @afox1029 - no problem - it's all pretty confusing. I found that asking questions about the hormone levels and lining also helped, it's not always just about the timing and follicle size. Sperm lives a while so having those little swimmers there and ready when all the other indicators are predicting success makes sense to me.
