@chiciphin When is your sono? Honestly I do think your
cervix kind of gets used to all the prodding and becomes less sensitive, which
is not very helpful. Maybe mention that your cervix is sensitive and can they
use something to numb it. Take some Ibuprofen beforehand too, that may help. Good
luck at your scan tomorrow.
@nitnat007 I'm so sorry this cycle didn’t work and you
didn't have any frosties. That just plain sucks so bad. I hope you are able to

afm... Blood draw and baseline US were yesterday. US looked good- “above average” follicle counts (18 and 17 on each respective ovary). HSG is tomorrow, eek.
BW looked good overall buuuut I have high prolactin. Retesting a fasting sample (a touch irritated that they didn’t just have me fast the first time- I was repeatedly told I didn’t need to fast). Still need husbands SA (which will likely be early next week) and our infectious disease profiles. He’s having genetic screen bw tomorrow AM then taking me to my HSG.
So of course I’ve gone down the google rabbit hole and have one million and one questions. (Interestingly could explain the short LP- ah ha!) I think once we have all of the puzzle pieces collected we meet again with the RE to start putting them together. What a roller coaster.

@crookshanks10 "What is natural cycle?" is a question I'm getting a lot! Even among folks who've done IVF it's a bit of a mystery. I will confess I'm still slightly confounded by the practical aspects of it, even though I get what we're doing in theory. It's probably easiest to explain it by comparing it to a medicated cycle transfer - in a medicated cycle the patient's cycle is controlled through medications designed to build a good uterine lining, and then when the lining and hormones look good progesterone supplementation is added. The patient does not ovulate at all, the conditions for implantation are created by the medications.
In a natural cycle transfer the patient does ovulate. There are two possible approaches to a natural cycle - one is to use stim medications or other follicle stimulants to help the patient ovulate and build a lining (@beachbunnyxo123 is using a stim type protocol for her natural cycle), the second is essentially no interventions at all, in the words of my IVF nurse "we just let your body tell us when it's ready". Both methods require as much if not more monitoring than a medicated cycle because the patient needs to both ovulate and have a thick enough lining to support a pregnancy. Using ovulation day as day 1, the transfer is done 6 days after ovulation, the time when most patients are receptive. Although the corpus luteum of the follicle that ovulated for that month produces progesterone to support the embryo, there is usually some other type of progesterone supplement, in my case it's suppositories, but it could be PIO for some.
In studies "natural cycle" transfers (which again, that term encompasses several protocol options) are approximately equal in success rates to medicated. Oftentimes in practice it seems RE's try a natural cycle when a medicated cycle hasn't worked, or where the patient's lining is not growing thick enough on the meds (stim meds, for example, often cause women to have better linings). My particular clinic actually sees a bump in success for natural cycle transfers. There are several us on the IF board December FET group who are doing natural cycle transfers, but I think I'm the only one who is using this protocol for her first transfer. The plus is lower intervention, the minus, at least in my case, is that you don't have a transfer date way ahead of time like in a medicated cycle. I've had three appts just this week, and I must have used 50 OPKs starting around CD10 because I was tasked with monitoring my surge at home as well.
For anyone interested -- Beat Infertility has a great episode this week (Bonus Episode 184) titled "History & Future of Fertility Science" and it's an interview with Dr. Michael Drews. It's a really good overview of the history of IVF, how IVF works, and some areas the science is heading next (like the study of mitochondria, which scientists now feel is the next frontier in increasing the success rates of IVF).

quick update for me - IUI was cancelled. It's CD9 and my right follie is 19mm. Schedule conflicts abound and MH and I can't get in to the clinic for an IUI before it most likely releases. So, medicated cycle it is. HIO once it is safe post-saline sono, aka tmrw night and onward. Should be interesting as we have house guests all weekend.

I also wondered what the difference was between a natural and medicated cycle, and what actually happens to the egg after you ovulate if you're going to do a FET. @chichiphin so sorry your IUI was cancelled after all that. AFM - we had some mixed news from the RE this afternoon. The good news is, the letrozole appears to be working and my biggest follie is currently measuring 14mm so he's pleased with where I'm at right now for CD13. Not so great news, MF's SA came back with a count of 19million and 66% motility. We got distracted talking about what that all meant before he could tell us morph so I'll have to ask him next time. Not great numbers, but could be a lot worse. He doesn't think we need to move to IUI just yet. Now for the bad news. I'm still processing some of this but I'm going to add a *TW* just in case.
They found a 5.9cm mass on my left ovary. He said it isn't a cyst so it could be two things - best case scenario it's just a fibroid but it's blocking my ovary on the scan so they just see a big shadow. Worst case scenario it's TOA/TOC but I have no history of PID and I've never had an IUD, no STDs, etc. He is sending me to a specialist for another U/S, and depending on what he finds, if they do suspect ovarian cancer I'll need an MRI and further testing. Problem is, MRI's have a 5+ month waiting list here in Canada. I also have a requisition for CA125 testing, which may or may not tell me anything useful.
I'm trying not to freak out because he said the chances of it being cancerous are really low, but he put that in my head so I'm a little unnerved. MF is terrified, he took it a lot harder than I did, it's the overprotective instinct in him. He said he hates that he can't protect me from whatever my own body is putting me through. I swear I cry more because of the adorable things he says than the actual news itself.

Unfortunately my right tube is occluded (left looks great). They even added a ballooning procedure and still nada. (That was fun...) The NP doing it felt confident it was spasming and not truly occluded since the dye stopped very cleanly at the entry to the tube but from what I can find, it looks like it'll need to be repeated in a month
Also had my infectious disease panel and fasting prolactin drawn today, but results aren't expected until next week. Still have to submit the SA and then get the results of the genetic screening (~3 weeks) before we can sit down for a follow up with the RE. It feels like a long time to wait but I'm reminding myself that if I hadn't advocated for myself, we'd still be blinding "trying" for another six months at the insistence of my gyn.
Before all of this I thought I grasped that IF sucked. Going through everything and finding more than one thing wrong really just makes me feel more and more broken. Hubs is amazing and reminds me that I am not in fact broken and I know in my rational mind that I'm not worth less as a human but gosh is it demoralizing. I appreciate each and every one of you and the support and kindness you offer on a daily basis

@chiciphin When is your sono? Honestly I do think your cervix kind of gets used to all the prodding and becomes less sensitive, which is not very helpful. Maybe mention that your cervix is sensitive and can they use something to numb it. Take some Ibuprofen beforehand too, that may help. Good luck at your scan tomorrow.
@nitnat007 I'm so sorry this cycle didn’t work and you didn't have any frosties. That just plain sucks so bad. I hope you are able to take care of yourself and that YH is caring for you. It sucks how busy this time of year is and how RE's office are slow to get those talking/planning appts on the calendar.
@swiftlet I don't think the drinking the night before would impact the sperm too much, but I don't really know. It might impact performance though…

@kobrien827 I hope the constipation has resolved some since your post. I used a suppository once or twice (I want to say DulcoEase) to resolve some of the constipation. Those really made it happen fast. That is a great 5 day report!

@kiki047 I'm sorry you're having side effects. I know with Met it is a bit better with food, and your body should get used to it some. If it doesn't I think there may be a slow release version that people like better.
@crookshanks10 I'm sorry the sample wasn't wat you expected. I've heard different things about morph -including that sperm with morph issues can sometimes work just fine. I hope you get a good result. I'm sorry the dr left you feeling pessimistic. Were those parameters very different than your other IUIs? Maybe it was just a bad day.

Blood draw and baseline US were yesterday. US looked good- “above average” follicle counts (18 and 17 on each respective ovary). HSG is tomorrow, eek.
BW looked good overall buuuut I have high prolactin. Retesting a fasting sample (a touch irritated that they didn’t just have me fast the first time- I was repeatedly told I didn’t need to fast). Still need husbands SA (which will likely be early next week) and our infectious disease profiles. He’s having genetic screen bw tomorrow AM then taking me to my HSG.
So of course I’ve gone down the google rabbit hole and have one million and one questions. (Interestingly could explain the short LP- ah ha!) I think once we have all of the puzzle pieces collected we meet again with the RE to start putting them together. What a roller coaster.
@zamora_spin It seemed to just be a bad day. At least his count was okay? Hopefully you're right, and morph is just one small piece of the puzzle! Oh my goodness, Tuesday is going to be here so soon!! What exactly is a "natural cycle" transfer? I am trying to learn as much as I can about IVF as that will be our next step.

In a natural cycle transfer the patient does ovulate. There are two possible approaches to a natural cycle - one is to use stim medications or other follicle stimulants to help the patient ovulate and build a lining (@beachbunnyxo123 is using a stim type protocol for her natural cycle), the second is essentially no interventions at all, in the words of my IVF nurse "we just let your body tell us when it's ready". Both methods require as much if not more monitoring than a medicated cycle because the patient needs to both ovulate and have a thick enough lining to support a pregnancy. Using ovulation day as day 1, the transfer is done 6 days after ovulation, the time when most patients are receptive. Although the corpus luteum of the follicle that ovulated for that month produces progesterone to support the embryo, there is usually some other type of progesterone supplement, in my case it's suppositories, but it could be PIO for some.
In studies "natural cycle" transfers (which again, that term encompasses several protocol options) are approximately equal in success rates to medicated. Oftentimes in practice it seems RE's try a natural cycle when a medicated cycle hasn't worked, or where the patient's lining is not growing thick enough on the meds (stim meds, for example, often cause women to have better linings). My particular clinic actually sees a bump in success for natural cycle transfers. There are several us on the IF board December FET group who are doing natural cycle transfers, but I think I'm the only one who is using this protocol for her first transfer. The plus is lower intervention, the minus, at least in my case, is that you don't have a transfer date way ahead of time like in a medicated cycle. I've had three appts just this week, and I must have used 50 OPKs starting around CD10 because I was tasked with monitoring my surge at home as well.
For anyone interested -- Beat Infertility has a great episode this week (Bonus Episode 184) titled "History & Future of Fertility Science" and it's an interview with Dr. Michael Drews. It's a really good overview of the history of IVF, how IVF works, and some areas the science is heading next (like the study of mitochondria, which scientists now feel is the next frontier in increasing the success rates of IVF).

quick update for me - IUI was cancelled. It's CD9 and my right follie is 19mm. Schedule conflicts abound and MH and I can't get in to the clinic for an IUI before it most likely releases. So, medicated cycle it is. HIO once it is safe post-saline sono, aka tmrw night and onward. Should be interesting as we have house guests all weekend.

AFM - we had some mixed news from the RE this afternoon.
The good news is, the letrozole appears to be working and my biggest follie is currently measuring 14mm so he's pleased with where I'm at right now for CD13.
Not so great news, MF's SA came back with a count of 19million and 66% motility. We got distracted talking about what that all meant before he could tell us morph so I'll have to ask him next time. Not great numbers, but could be a lot worse. He doesn't think we need to move to IUI just yet.
Now for the bad news. I'm still processing some of this but I'm going to add a *TW* just in case.
They found a 5.9cm mass on my left ovary. He said it isn't a cyst so it could be two things - best case scenario it's just a fibroid but it's blocking my ovary on the scan so they just see a big shadow. Worst case scenario it's TOA/TOC but I have no history of PID and I've never had an IUD, no STDs, etc. He is sending me to a specialist for another U/S, and depending on what he finds, if they do suspect ovarian cancer I'll need an MRI and further testing. Problem is, MRI's have a 5+ month waiting list here in Canada. I also have a requisition for CA125 testing, which may or may not tell me anything useful.
I'm trying not to freak out because he said the chances of it being cancerous are really low, but he put that in my head so I'm a little unnerved. MF is terrified, he took it a lot harder than I did, it's the overprotective instinct in him. He said he hates that he can't protect me from whatever my own body is putting me through. I swear I cry more because of the adorable things he says than the actual news itself.
