Re: July 2018 IVF

  • @thebabyshoes thank you for your thoughts on natural vs medicated FET. We haven’t talked through the options yet with our RE and the background is helpful. Fx your beta goes well!
  • Hi ladies! I wanna thank you all first for your all your support and helping me going through this. So my retrieval went well, still recovering. They retrieved 33 eggs and will call today or tomorrow I think to say how many were matured and fertilized. But another hiccup happened

  • So my DH gets really nervous when it comes to give a sperm sample. So while I was doing my ER he was supposed to work on getting the sample. He gave them a pre-come apparently because nothing could come out. So after working on it they came back to him and there were no sperm in that sample that he has to try it again. So he did, and result came out still no sperm. So I that point I was out and more alert, and we started getting concerned. RE suggested to try again and if nothing we will have to freeze the eggs and DH will need to see a specialist for treatment and he doesn't know how long that could take. So we tried it again then bam we got the good sample like he always had in the past. That was craaaaaazyyyy stressful! But glad we got good sample to have the lab work with. I will keep you posted when I hear from the clinic. @lemondrop1976 glad to hear that the 2nd one fertilized as well, GL to you! Keep us posted. And welcome @thebabyshoes!
  • @babydust2507 glad to hear things went well today, though sorry you both had to go through a bit of a scare to get there! We go through all the injections/hormones/etc, but I cannot imagine what it’s like to be in a man’s shoes when needing to give a sample on command, I would so fail... FX for a good report tomorrow!

@lemondrop1976 maybe it's a day 1 transfer for u. Ours is also day 5 ( blast stage) transfer.

@anared I started my period on 23rd - 12 days after retrieval. I am on BCP from 27th, starting lupron on 14th August. Estrodial on 25th transfer on 12th. From retrieval which happened on July 12th it is 2 months for me. I think there will be different protocols, mine probably is the longest.

@babydust2507 congratulations on those good numbers. Sorry u had to go through so much stress last minute. It is always stressful if there is clock to it.

@thebabyshoes welcome and good luck for your beta. When is it?
  • Wow!  I missed a lot over the last few days!!  Thanks to everyone who checked in on me

@kbenner2 I hope everything is still going well for you!

@mackorori that is great that you were able to biopsy 6 for testing!  Keep us posted on the results.  I am sorry if you mentioned this already, but do you have thoughts on when you would like to transfer?

@bkatp I think they will typically biopsy when they are fresh for PGS but I have heard of people banking the embryos (doing retrievals and freezing until they have enough) and then doing the biopsies.  I think there is certain criteria that REs will follow though as to who they recommend PGS for and I am sure they would have let you know if you were a good candidate, so I am sure you are good!   Fingers crossed that this FET will bring you the good news you have been waiting for!  Are you happy to have your calendar and a plan set?

@babydust2507 I hope that everything with your retrieval went well today!!

@lemondrop1976 Fingers crossed that they are able to fertilize the egg which is still maturing.  Good luck with your transfer tomorrow!!  Here's to hoping that this is it!!

@thebabyshoes welcome and good luck with your beta tomorrow!!  Keep us posted!

@anared Congratulations on your embryo!  I know the feeling though of starting with so many embryos and then watching the numbers dwindle with every step.  My numbers weren't that far off from yours.  Try to stay positive.  This little embryo could be the one!  I look forward to seeing you on the FET boards in the fall with me.

As for me, we FINALLY got our PGS results back.  Overall, they were able to retrieve 18, 12 were mature, 10 fertilized, 7 made it to day 3, and then on day 5 (biopsy day) we were down to 5 blasts.  Of those 5 blasts, 2 came back normal.  For the 3 which were abnormal, we had one with an extra 16th chromosome, one with 2 X chromosomes and one Y chromosome, and one with "chaotic chromosomes" or mosaic.  It feels like it has taken so long to get to this point.  I am looking forward to being able to start focusing on transferring!

Beta came out negative. I'll crawl out of my dark cave is despair in a bit.... I lost my only healthy female embryo so I'm feeling really devastated.

    Beta results

  • @katie35 congrats on getting your results back! Wishing you the best for the FET process
  • Sorry to hear @thebabyshoes, I know how terrible you must feel which cannot be put into words. Stay strong.
  • I'm so sorry to hear that @thebabyshoes, stay strong and I'll be praying for you
@katie35 ER went well got 33 retrieved I will know tomorrow how many where matured and fertilized, and congrats on the good PGS results! are you going to transfer the boy or the girl?
  • @thebabyshoes I’m so sorry to hear today’s results and keeping you in our prayers. It’s such a struggle sometimes.

@katie35 I was wondering how things were going, very happy to hear you have the 2 normal embryos! When are you planning FET? We are going to Kauai at the end of September, so taking a couple of months to settle down from the hormones and figure out a calendar for after the trip. I’m guessing Oct or Nov, we will regroup with the RE once our results come back.

@lemondrop1976 good luck with your transfer tomorrow
  • GL @lemondrop1976 with your transfer! will be praying for you!
@mackorori that's a pretty nice plan! enjoy your vaca!
  • @thebabyshoes I am so sorry to hear of your BFN.  That is so hard.  I will say a prayer for for comfort for you.
  • @babydust2507 that is a great number!  Fingers crossed for a good report today!!

@mackorori I think that sounds like a good plan.  Let yourself relax for a couple of months and a good vacation sounds great!  We are planning to transfer hopefully in October.   We are going to do an endo scratch next month and then transfer the following month.  More waiting but I think a little break will be good for me too.
  • @thebabyshoes sorry to hear about BFN. Please stay positive and strong
  • @katie35 good to hear back from you. Yes I am happy it's going ahead but they keep saying if everything looks good. So I have to to be careful in getting excited about the date.
  • thank you @katie35! the wait is killing me tho! I didn't even go to work today, I decided to work from home:/
@bkatp I know exactly how you feel, felt the same way they kept on saying that because of my high risk OHSS, I thought it was a way of them warning me that my cycle would be cancelled. Hang in there and FX!
  • My transfer was this morning, so I was surprised with an extra embryo. They transferred 3, although they were not rated as highly as the last cycle month's 3 (which were all level 4, 8 cells and compact).
This time all level 3 and fragmented (5 is best at this RE) and
    2 were 6 cells
    1 was 4 cells
So here's to beating the odds again;)
  • @lemondrop1976 what a nice surprise, hoping for the best!!
  • congrats @lemondrop1976! that's promising! FX and pray that exciting news will keep coming your way!
  • @lemondrop1976 an extra embryo, that is great! fingers crossed!
  • @katie35 thank you! and congrats with your two embryos! Oh I know, it feels like it took a lifetime to get to this point and still have more to do. I look forward to the fall FETs!

@thebabyshoes I am so sorry :( lots of hugs

    @bkatp thanks!
  • so I finally got an update from the clinic, out of the 33 retrieved, 21 were matured and 14 fertilized. The plan was at 1st to do 50% of the matured a conventional fertilization and 50% ICIS but because of the hiccup that we had with my DH sperm on the 1st 2 tries they decided to do ICIS with all of them, and I am not mad at them for that! that's what I wanted to begin with, I didn't wanna waste a entire batch if conventional wouldn't work for us. So I will be getting a call regarding the quality of the embryos and when transfer is going to be.
Thank you everyone for checking on me!
  • @babydust2507 Great numbers, best of luck!
  • Great to know you got an extra embryo. More probability now to BFP. Good luck @lemondrop1976

@babydust2507 congratulations on great numbers. Your transfer is coming fast, are you freezing the rest ?

How many are you transferring or decided to transfer 1-2 ? I am faced with that question now, I have to decide in next month on what my number should be. I want 2 just to increase the probability but they have given a caution list with transferring 2 eg: premature delivery, high blood pressure, gestational diabetes and under development of babies. The list scared my DH and he gave them 1 as the number. But I am still inclined to 2

    What are your opinions?
  • @babydust2507 I'm happy they did all ICSI! Our clinic typically performs ICSI which I think is good because that way all the embryologists are well trained since that's mainly they do. We had a client who requested a fraction of her eggs with conventional and NONE of them fertilized while the majority with ICSI did. I think with conventional the sperm have the added task of penetrating the zona and it's hard to say if that's one of the underlying factors for their infertility to begin it. With ICSI you bypass that barrier.

  • Exactly! Totally agree @embryoman! I guess everything happens for a reason!
@bkatp, I will be transferring only one and freeze the rest that will make it to blast. And yes, they told me about the risk of transferring more than one. And my DH is also scared about things like that so we told them one and moved on and hope for the best. GL to you!
  • @babydust2507 glad to hear great numbers, what day will you do the transfer?
  • @babydust2507 congrats on the great numbers! ICSI is definitely the way to go!

@embryoman that was our issue with low morphology, none of the IUIs worked. All of our mature eggs fertilized with ICSI, although we only ended up with one normal and two low level mosaic, it is the closest we have come since starting.
  • Thank you @mackorori and @anared!
@mackorori I don't know yet the day of the transfer,l. I think they said it will depend on the quality of the embryos if they will do on D3 or D5 after retrieval. They're supposed to call me today to clarify that so FX!
  • @bkatp I'm not sure about your exact situation, but when deciding how many to transfer I know that your age and expected egg quality play a big part in the decision. In my case I'll be 42 and have poor ovarian reserve so they would consider transferring up to 5. I had 3 great 8 cell embryos last month transferred and ended up with no implantation which could be due to egg quality so with me its trying to get that needle in a haystack to come along. Im hoping one of the 3 this round beat the odds.
  • @bkatp I agree with @lemondrop1976 age and eggs quality play a good factor on how many eggs should be transfer
@kbenner2 how are you feeling? I know you had 11 that fertilized and I am not sure if you already mentioned it, if so excuse me to ask that question again, but how many made it to blast and how many did you freeze? Also, i would like to know when did you start feeling sick (the OHSS symptoms) before transfer or after? I was feeling ok yesterday but still bloated and gained 2lbs, but today I can feel the cramps again like the day of retrieval but maybe not as bad. But I lost 3 lbs. Of course I will ask my RE but was trying to see if that was a beginning of OHSS
  • Thanks @lemondrop1976 and @babydust2507. I am 32 and I have 3 - 4B+B  2 - 4BB  1 - 3BB embryo's My clinic said they wouldn't give an A for any of their embryo's and stop at B+.  . I know a good quality is 4AA, in my clinic's case 4B+B+, That being said none of mine are 4B+B+
  • The unfortunate reality of age and egg quality hit home today...in spite of having a better ER cycle with more and better quality making it to blast, all of our 6 embryos came back abnormal. I know the statistics say it can happen, but that doesn’t make it easier. We have our one normal embryo from the first ER, so once I give my body a break and take a much needed vacation we will hopefully have a successful FET with that one.
  • @mackorori keeping you in my prayers and that one embryo will be the one! sorry for that bad news...
@bkatp seems like your clinic is pretty harsh grading and I'm pretty sure the grades that you have are good enough to give you your BFP.
So I got the report from the Lab today, so all 14 are growing the same pace which they said they were happy about it 4-6 cells and graded B. I don't really know their grade scale at the clinic, this is really new to me, I didn't even know at first what you guys were talking about when I saw all these letters on your posts (LOL) kinda make sense now so I will ask tomorrow, but they said B grade is what they are expecting at this stage at their clinic, so I'm guessing I'm still on the right track. My transfer will be Sunday. Also, the cramping that I was having today was from constipation. I started feeling better after a bowel movement (LOL). I talked to the nurse about it and she said to use Colace, that the constipation was due to my high estrogen, the anesthesia and also the pain medicine I was prescribed after the retrieval. Hope I will feel better tomorrow.
  • @lemondrop1976 @bkatp For embryo transfers...Our clinic typically follows ASRM guidelines (see table 1 in the link below). They're regarded as the official guideline on the # to transfer.

    https://www.asrm.org/globalassets/asrm/asrm-content/news-and-publications/practice-guidelines/for-non-members/guidance_on_the_limits_to_the_number_of_embryos_to_transfer-norpirnt.pdf

@babydust2507 I hope you feel better! I can share my experience in grading embryos. We have an A/B/C system. It sounds like yours has a "+" level as well which is interesting so they must be pretty particular!  A is good, B is fair, and C is poor. Sometimes "D" is used as well. Depending on the stage of the embryo this can refer to different things. When the embryo is cleaved, it usually refers to the amount of fragmentation (how much the cells break up into smaller pieces), the "usual" cell number (you expect at least 6 cells on day 3), and the symmetry (do the cells divide on the right plane). After that is a morula which kind of follows the same system. Then it's a blastocyst that has the inner cell mass (becomes the fetus) and trophectoderm (becomes the placenta), and these receive letter grades as well depending on the cell number and fragmentation. Grading is pretty subjective and the angle you look at the embryo when you do the check is not always representative of the entire embryo. They're little balls that can be spun around and sometimes they look worse on one side vs another. So be wary of grading! If you have a blast you're in a good place, I think!

  • @babydust2507 I’m feeling a ton better thanks for asking! Almost all bloating has gone down... thank goodness because when we start school this week (I teach 2nd grade)I did not want to look 6 months preggo when I’m only 6 weeks!!! Haha!!! As for my embryos... I only had one of the 11 fertilized make it to the blast stage by day 5 transfer. This embryo was graded 3 AA and they said they hardly ever give out A’s at my clinic. The other embryos were still growing but not ready yet so they were going to watch them for another day to see what they could freeze. We were definitely bummed because we planned on transferring 2, but everything happens the way it’s supposed to! Unfortunately none of ours were good enough quality to freeze which was devastating to me at the time, but a blessing for us now since we are pregnant.  My husband and I were very concerned about having left over frozen embryos during the ivf process due to this being our last child. Anyways... our clinic only does 5 day transfers. As far as the OHSS goes... I didn’t have any symptoms after egg retrieval at all! It wasn’t until about 5-6 days after transfer that I started getting bloated and short of breath. Do you know when your transfer is yet?

@mackorori I’m so sorry about your embryos. That’s such hard news to take in. This other little embie will be the one.... praying for you.
