CD3 test results inside (not amazing) + question (TW: child)

After an unusually long cycle (50 days), I finally had my CD tests and just got the results.  We are TTC#2.  I'd love some thoughts on my prospects from the veterans on here.

The doctor who did my ultrasound described my AFC of 7 as "low to normal" and "not unusual for my age (almost 40)." Before we did the tests, the RE was very encouraging in saying that "whatever numbers you get, your past success is encouraging; the numbers do not tell us much about a fertile population." So of course I am hanging on to those words, lol. But I'm also slowly coming to terms with possibly being "one and done."

If anyone has comments on the results, please chime in. I don't know much about any of this beyond the ranges listed on various websites.

I'm also wondering if these results point to any particular protocols v any others.
AFC 7  (plus one cyst that she said could be hiding another follicle)
AMH 0.98 (1 is "low normal," so I'm going with normal, lol!)
Estradiol 37.8 (seems fine?)
LH 5.8
FSH 11.9 (high)

I skipped HSG because I want to avoid the extra radiation (I already fly a lot long haul for work and am worried about my annual exposure).  I haven't yet spoken to my RE about this, but will try to do so this week. If anyone was able to skip the HSG, I'd love to hear about that too.

Thanks so much.

PS I posted a similar message in TTC>35 but that's a quieter board these days...

Welcome @marijaa333!  How long have you been trying for #2? What did your doctor say once you got the numbers?

It looks to me like your numbers are all borderline.  And an AFC of 7 is low, but not terrible.  Did the RE say what the minimum number of follicles is before they’ll cancel a cycle?

Regarding an HSG, I believe insurance requires it as part of the evaluation process, sonyou May want to look into that.  Also, for me it actually was very important because I had tubal scarring which was the reason for my fertility.  I ended up having my tube (I only had one left because I had an ectopic in November of 2015 that resulted in losing the other one) removed before IVF to reduce the chance of fluid backing up into the uterus and also reduce chances of another ectopic.

Good luck, I hope you have success!

  • @mayflower513 Thank you! And nice to meet you. We've been trying for 8 months, and I'm 39 (40 in early October). #1 was conceived quickly.

No, the RE didn't give me any minimums nor have I been in to discuss results yet. They just showed up on the patient portal. I have to call her about the HSG first. I was afraid that insurance may require it, but I would really like to avoid it if possible.  I have heard an alternative is a sonohysterogram (a special ultrasound), so I'm hoping that's an option.

In case it is relevant, I would prefer not to try IUIs, clomid, etc. but move straight to IVF due to age...   I will talk to the RE and report back on this thread.

Thanks again for chiming in!  I hope you are successful very quickly as well (I will lurk on the other threads now to see what's happening with y'all).

  • @marijaa333 My RE sometimes allows patients to do a saline sonogram instead of an HSG. Saline sono is done in the office and they put fluid up in your uterus and look at it on the ultrasound. It does not show the tubes in any way but my RE says it gives a bit more detail of the uterus. You could ask your dr about that to see whether that would be appropriate given you want to move right to IVF. I’ve had both an HSG and a saline sono and neither were a big deal - the first time around I wanted the HSG to make sure my tubes were ok, although in my specific case my RE said I could do saline sono instead. In any case before IVF they will require one of the two.

On your numbers it seems to me with the higher FSH and borderline AFC and AMH, plus your age, moving to IVF seems like the right decision although I know that’s hard to come to. Good luck!

Just to chime in on what @zamora_spin said - see about the Saline sono. It doesn’t give a great picture of your tubes but they can tell if the saline fluid is able to be pushed through the tubes indicating they are likely clear. . It’s the test my clinic would run before moving to the HSG.

  • Thanks @zamora_spin and @JWatt5 - great to know about the saline sonogram, I will definitely call tomorrow to ask (that sounds the same as what I had heard about before under the term sonohysterogram).

And yes, I would prefer to move to IVF right away.  With my age and numbers, and based on what I've read, I'm estimating my chances of everything working out for an IUI to be pretty low.  Although, I continue to be hopeful in the next few months before we can get there...
