Day 3 Embie Cell Development Questions

Hello 

I spoke to the Embryologist this morning about our 3 fertilised eggs that were fertilised with ICSI. 

She said that the embryos were all doing well and were being frozen today. She said we had an 8 cell, a 10 cell and a 12 cell, and that some were already advancing to the next stage of development. At the time, I thought this must have been a really good thing, so I was quite stoked. Now I have realised the quick development may not be such a good thing, and I wish I asked more questions at the time. 

Does anybody have any experience on this?

  • I would say if they were worried, they would transfer at 3 day instead of 5 or freeze if you're doing pgs. This is just my assumption!

  • On Day 3 I asked my embryologist about "what's a good number of cells", and he said - anything from 6 to 12 cells is good for Day3. Yours are right in the range. Also, I read another believe somewhere the even number of cells on Day3 is a good sign. :) I'm very excited and optimistic for your embryos :)

    Sept 2020 FET#3: one XX embryo, BFP, Beta#1 (9dpt) - 161, Beta#2 (11dpt) - 519, Beta#3 (19dpt) - 7174, Due date 05/30/2021

    Thank you all for replying ☺️

    We had to do a freeze all due to risk of OHSS, and they didn't want to risk going to 5 day in case we ended up with none (I kinda believe getting them to 5 day weeds out the weaker ones that never would've made it to babies) but I guess it would've been disheartening potentially ending up with none as well. 

    I have to call them today, so I'll ask a few questions and keep you in the loop ☺️

    Spoke to embryologist again this morning. 

    My gradings were actually: 

    1 8 cell grade 5 (I think our grading is different to ?US as the higher the grade number the better rather than the other way around) 
    1 10 cell grade 4
    1 11 cell grade 4 

    He said he would actually rather see the embryos developing more cells, and that we should be quite pleased with the grading/outcome :)

    Knottie34543584 - great news! I think those numbers are rocking! My clinic doesn't assign numeric grades to embryos, just the "good", "fair", "poor", and all of yours would be in "good" bucket :) Did you have a discussion for when you'll be doing FET and how many they'd put back?

  • 2/6 of my embryos made it to day 3. 1-8 cell, 1-10 cell. We implanted both hoping that they would do better inside. Doc didn't want to risk both arresting before transfer.

    @FuzzyDust That's a bit annoying they don't grade them a little more, but probably not such a bad thing for overanalysing
  • @FuzzyDust Weird it deleted most of my post!! But thank you very much :) yes, she said once I start my period I will start a medication for 10 days (I am guessing estrogen) and then progesterone suppositories, and I'm sure she said one other medication 10 days prior to transfer? But she said the whole thing takes around 2 weeks and just mimics a natural cycle. Hoping af will show up soon! How is everything going for you? X

  • @Knottie34543584 - For me AF showed up on Day 10 after the ER. It's heavier than usual, but I had way thicker lining during IVF than what I had before (during IUI, and I don't get AF at all if I don't take BCP). It's a little sad to see this beautiful luxurious lining being wasted  :(
    I'm still waiting for the genetic screening results, hopefully by the end of week, or early next week. I don't think we'll be doing FET right away at this point, but since I don't have a cycle, I don't need to align with any timeline.

    Did you discuss with your doc how many you're putting back? I think it's usually more if it's Day 3 embryos.

  • @Knottie34543584 - My RE's FET protocol sounds identical to yours! Starting with estrace tablets, a monitoring appt somewhere between 10-14 days to see if lining has gotten thick enough...then will start PIO with the transfer on the 6th day of injections. I think they will also have be take doxy or some antibiotic for a few days before and after transfer. The only question I'm waiting on an answer for is if I can start this as soon as I get AF that comes after ER, or if I will have to sit out a full cycle first...

    @FuzzyDust - I remember thinking the same thing about my lining during IVF! Even though they knew I was freezing all, the ultrasound techs still measured the lining at each appointment, and it was so much thicker than normal (i think)!! FX that you get most or all of your embryos back tested normal! :smile:
  • @FuzzyDust I know! Such a waste isn't it. But good to see that your body is able to build a great lining :) last time AF came 5 days after retrieval, but I think Decapeptyl quickly fazes everything out so I reckon that's why it came so soon, I imagine HCG trigger holds out luteal phase a bit.

    They can only put one back in at a time. I'm not sure if it's state or national regulation or just because of my age group, but it's a bit frustrating! He said he can get in big trouble if he puts in more than one a time, but they will put in 2 after 5 successful rounds. 

    Goodluck for your genetic testing results, I really hope you have great news!! I can imagine you are feeling anxious. 

    Interesting @caseyw8784 it does sound very similar! Mine are oral medications and suppositories I believe. Hopefully you don't have to sit out a cycle, but if that's what they recommend it'll be worth the wait :) I am surprised my clinic allows everything so back to back, but it is handy. All the best!
