yet another question- UTI edition

ok. bringing another dilemma to the table here.... quick condensed backstory:

i am 11w6d. Had spotting from 6w-9w (sch)

went to ER at 7 weeks and they said I had white blood cells in urine (uti) and gave me Macrobid. Made me sick as a mofo, but finished the week. 

Started noticing some urgency/frequency with little output, coupled with some pressure and cramps feelings again about a week and a half ago. Took an AZO test strip and was pos. for leukocytes (neg for nitrates)  called the OB, she called in another Macrobid script which was surprising since clearly it didn't do the trick just a few weeks before that. But she said there aren't many safe options. 

Another week of macrobid violent nausea hell. But zero relief in symptoms, still getting a flaming positive on the AZO strips. Now 2 days ago I began spotting again, with cramps. Called the OB again and they jus said to drink a lot of water and cranberry until my appointment on Tues. so she can send out a culture and if it gets worse go to ER. I was less then thrilled with that but, ok. Now spotting is getting somwhat worse (not red) and I know that can be normal blah blah blah and more pressure. I used the Doppler this morning and there is a HB but, my pgal brain has me pretty on edge. I know it's not really an ER emergency but, I am nervous to go untreated for much longer since symptoms and spotting are increasing. I have an appointment on Tues but she doesn't want to give me anything else until a culture comes back and that's a few days from then.... and this will likely be at least 6 weeks now with a stubborn, festering uti. Any advice?!

  • I don't have any advice but I know just how awful UTIs are. I had them like once a month or so before I had DS. I would wake up and it would sting to pee and then by the next time I was peeing blood. Just awful. Cranberry juice never helped but if I drank my weight in orange juice that seemed to help, I read that the orange juice and vitamin c and stuff helped make it too acidic for the bacteria. Maybe it was just a coincidence but it helped me. Also I had a UTI when I got pregnant with DS and they gave me medicine for it but I don't remember what it was called, I think they were yellow and gray capsules but I really can't remember! :( I hope your UTI gets better. They're so awful! Do you have any urgent care or walk in clinics in your area?

    I had several UTIs with my second pregnancy, albeit without symptoms at all.  I feel like they gave me a pretty standard antibiotic to treat, but I don't remember what it was.  But if I was in your shoes, I'd go to an urgent care center or ER to get checked.  It really might not be anything, but peace of mind is worth a lot to me.  And it certainly can't hurt.  Nothing wrong with getting a second opinion.

  • No UCs. It's just frustrating. I am drinking a crap ton of cranberry which is killing my reflux. I also have a bicornuate uterus which is usually linked to having only one kidney (which was due to get checked at my next u/s)... so, I'm just concerned of infection while trying not to be overly dramatic but, I was told by the ER doc that UTIs can cause miscarriage so I'm just keeping calm over here like  :#

    And I think now bc of the antibiotic I have a YI which is just lovely. I'm telling ya if it's not one thing it's something else. When does this start getting fun again? Lol

  • I could drink cranberry all day and it wouldn't do anything :\ orange juice was the only thing that helped and I had to drink ridiculous amounts for like two days. But since you've had it for so long and your spotting is getting worse, I'd probably head to the ER since you have no other options like an urgent care or a clinic.

    You can take augmentin and keflex (unless your allergic obviously). Those are options while pregnant too with a UTI. I agree, if macrobid didn't do the trick the first time it's not going to magically make it better the second time. So sorry you are miserable! I would call your OB back and tell her you are not feeling better. 

    Edit: just re read your post. If no relief in symptoms after a week of macrobid and your at home test still showing positive leukocytes head somewhere to get treated. That's crazy they are going to make you wait until Tuesday and THEN send off a culture. nope. I would not be ok with that. 
  • I just got over one, and had one my first pregnancy as well. Amoxicillin worked for me. I stopped wearing tight clothes, and *TMI* no underwear at bed. Hope you get some relief! @OmSweetOm

  • Thanks everyone! think I will head to the ER first thing in the morning. I'm honestly too damn wiped out to go now and sit for hours plus DH is working late and I reeeeeally don't want to go sit for hours alone but, I also don't want to put it off either. I agree it's just way too long to wait even if it is a few days away. The spotting is what really bothers me. It puts a really bad taste in my mouth that she knows I've had a nonresponsive UTI for several weeks now and just told me to drink cranberry juice.

  • I recently read that drinking cranberry juice to treat a UTI is a bunch of baloney. I can't believe yours is so bad that you are spotting and your doc told you to just drink cranberry juice and wait! And to give you the same antibiotics that made you sick and already didn't work?? That's nuts! I'd be so mad. If I didn't have an urgent care by me, I'd definitely go to the ER if I were you. Especially if you are pgal. I feel so bad for you! Having a UTI while you ARENT pregnant is the worst. I can't imagine how much more terrible it must be for you. :(

  • I have no advice, but I agree that you should be seen before then. UTIs can be
    horrible even when you aren't pregnant... hope they have some answers for you!

  • I wold recommend going to an ER instead of an Urgent Care. I get a few UTIs a year, and have learned that most Urgent Cares aren't equipped to do cultures, only dip tests. (You can diagnose a UTI by just dipping a strip in a urine sample, but a kidney or yeast infection need a culture.) I had a kidney infection lats year that went untreated for a week because the UC failed to tell us they weren't equipped to diagnose them.


  • Thank you! This makes me feel much better because I was very hesitant to go to the ER because I was nervous it wasn't serious enough.. but that is a good point.

  • It depends where you are but some walk in clinics can send off cultures, but they take 2-3 days to come back. 

    Cranberry juice has some contradictory studies. The thought process is that it decreases the pH of the urine and bacteria can't adhere to the wall. However, it will NOT treat a UTI. I can't believe your OB told you that either. Give us an update! I hope you get some relief soon. 
  • I have always taken a supplement called D-Mannose when I have had UTIs. It is an extract from cranberries that attaches itself to bad particles in the urinary tract and flushes them out. I haven't had a UTI pregnant, so definitely ask your OB if it is safe. It was always as effective for me as antibiotics but I never had side effects and didn't have to go to a doctor for it.

  • Definitely a good call to go the an urgent care or ER. It might be worth calling the UC to see if the do the culture so you can avoid the extra expense and/or wait of the ER. 
  • @OmSweetOm that's so good to hear that they gave you stuff to help the UTI and the YI! And I'm so happy your little babe is doing well. I'm sorry you have another SCH, hopefully it resolves soon! Thanks for updating, so glad you went and got it taken care of! Now sending lots of thoughts and good vibes that this medicine actually works. 
  • @OmSweetOm there is so much resistance to UTIs lately it's getting bad. So glad you went and got checked out! Hope you start feeling better soon.
