TTC After a Loss

Progesterone and baby aspirin

Meet with my OB to discuss a realistic picture of what I'm looking at on the ttc front. He told me that the minute I get a bfp again to call him and he's going to start me on progesterone and baby aspirin. Had anyone else done this? I know the reasoning behind it, I'm just wanting to hear how it went for people that have done this.

I had my 2nd mc on 1/9. I turned 35 on 1/6.  So between the two neither of us want to take the wait and see approach. My progesterone doesn't appear to be an issue and I do have a rainbow baby between the losses but he wants to prescribe it as a preventative measure just in case. I was happy that he suggested all the things I went in prepared to ask for. Being on the same page made it nice.

  • I haven't done this yet, but this is my RE's standard practice for a BFP after RPL. So, I'll be doing this hopefully soon! Her opinion is that while the evidence is mixed on whether it helps, it can't really hurt. And it will help me to feel like I'm doing something different. So, we're going with it.

    I had two miscarriages 6 months apart in 2015 and I took baby aspirin for my entire pregnancy. LO arrived heathy almost 4 weeks ago. Best wishes to you!

  • I am in the same boat after two losses with instruction to call as soon as I get a bfp. So I'm not sure what to expect.

  • I've never heard of it from my OB yet.  We are going to check my progesterone level later this month.  I'm assuming if it comes back low then I will be put on the suppositories while TTC and up to 10 weeks pregnant.  Otherwise if my level is fine I don't think I will be told to take it.

    I had two MCs in 2015, back to back. I knew once I got another BFP, I would be put on progesterone and baby aspirin. I am now 27 weeks along with a healthy baby. I stopped progesterone at 12 weeks but will take aspirin the entire time. There wasn't anything to suggest in my blood work that I needed these things but it's a precaution that can't hurt, as RiverSong mentioned. Best of luck!

    @vlagrl29 Aspirin is non-steroidal anti-inflammatory drug (NSAID). So, it's helpful at reducing low levels of inflammation. It also is a blood thinner (that's why it's often recommended to people with heart disease or who are having heart attacks). Baby aspirin is just "low dose aspirin" (81 mg). So, if someone has low levels of inflammation or a mild blood clotting issue that may cause problems in pregnancy, it's recommended. As I said above, studies are mixed. Some have found an increase in the live birth rate, others did not. I had slightly elevated antibodies related to an autoimmune blood clotting disorder (so, inflammation + blood clotting related) - however, my bloodwork result wasn't enough to diagnose or be an official problem, but baby aspirin may help offset that if it is a problem, however unlikely. As I said before though, it's more of a "it can't hurt, and it might help, so try it". It's definitely not something most women need, even women who have had one loss (as opposed to recurrent losses). 
  • This was my first loss, but my doctor will be testing my HCG and progesterone the day of my next BFP with follow-ups, just in case I need to be put on progesterone.

    I have a 17-month-old and didn't have progesterone issues with his pregnancy, and don't have the other symptoms of low progesterone during my cycles, so I'm pretty sure that's wasn't the issue with my loss, but I would rather have it tested right away next time than be sorry! *End TW*

    I sure hope I never need baby aspirin considering I'm allergic to NSAIDs!
  • @jen83mn- I didn't have progesterone issues either with DD.  Granted this was about 6 years ago.  I never even had it tested.  I just want to know before getting pregnant again what my progesterone level is.

    I will probably need both considering my temps were all over during my pregnancy (suggesting low progesterone) and possibly blood / autoimmune disorder. Still waiting for MFM to see if they will do bloodwork and don't want to mess it up by using either yet, but I have already bought the baby aspirin (was told I might need it two weeks before my MC due to abnormal bloodwork) and natural progesterone cream (not sure it would be enough) in case they play it cautiously again without testing (although I think I might demand it this time if they don't suggest it).
    It's funny how it's the little things in life that mean the most...not where you live, or what you drive, or the price tag on your clothes... There's no dollar sign on a piece of mind, this I've come to know! *ZBB*

    I have been on baby aspirin for a couple months as recommended by my RE. As soon as I got my BFP, he put me on progesterone supplements because, like Riversong15 said, it can't really hurt. My progesterone with my previous CP was low and with my MC last year, was borderline so I suspected it might be part of my problem.  Of course, it's kind of "chicken or the egg" on this. Was the MC caused by the low progesterone or was it low because it was nonviable? Can't really say.

    I would say neither can hurt and the progesterone is typically only until 10-12 weeks. I had 2 losses back to back in 2016 and it was found I had slightly elevated anticardiolipin igm- can cause blood clots up to 20 weeks (blood vessels widen in 90% of women thus making it no problem for igm positive). The baby aspirin thins your blood to prevent any clots to an otherwise healthy baby. I'm also on Lovenox shots, not awful just annoying. Progesterone pills are also good, keeps your levels up until the placenta takes over. Mine were on the low end of normal so they did it as precautionary because it can't hurt and only help. My one piece of advice would be early u/s to rule out etopic if your on progesterone, doctor should do this but if he doesn't suggest it- ask for it at 5 weeks to check the sac is in your uterus. Good luck and so sorry for your losses. Your doctor sounds good & proactive. 
  • Been lurking so far on this board, but wanted to comment and say I am wondering about this too. I have always had spotting several days before my period, and had no idea this could be a sign of low progesterone until I was reading about it yesterday.  I also didn't have as much bleeding with my m/c as some women, not sure if this could be a sign of low progesterone (like maybe the lining wasn't built up enough to support). I am going to ask my OB at my follow up appt to test and I hope she will. I have heard so may success stories.

  • @liz4382- the spotting can or can't be a sign of low progesterone.  I've spotted for as long as I can remember and I have 1 living child.  I'm going to get it checked just for a peace of mind.

  • @vlagrl29 good to know.  I have always had it too and never gave it a second thought.  So maybe it is nothing--but I agree that it is better to have it checked and get some peace of mind!

  • Interesting read. My doc said at my follow up to my CP that she wasn't going to recommend baby aspirin yet since it was my first loss. But I do have a bottle and thought about taking it. 

    So from what others have read, there isn't generally a concern of it doing harm, right? 

    Also, when do people typically take it while ttc? Entire cycle or after O?

    I don't have any confirmed blood issues, my main problems right now is that I have been having 1.5 day periods...so going in later this month to see if anything is going on.
  • @dragonfly87 I decided to use baby aspirin on my own with my last pregnancy.  I asked my midwife about it to confirm that it was fine, and she assured me it was.  I still lost that pregnancy but the aspirin wasn't the cause (that loss was identical to the previous two when I hadn't taken aspirin).  My OB said we will probably try aspirin and heparin next time although we're still waiting on tests to try to find an answer.  I still take one baby aspirin every day throughout my cycle.
